摘要
目的比较严重心肌梗死患者经心脏超声、13^N-NH3·H2O和18^F-FDG心电门控正电子显像(GPET)测得左心室射血分数(LVEF)的差异,探讨心肌梗死后13^N-NH3·H2O和18^F-FDG GPET测得的左心室功能指标的相关性和一致性。资料与方法回顾性分析50例心肌梗死住院患者,于冠状动脉介入治疗前进行GPET心肌存活评估和心脏超声检查,比较3种方法测得的LVEF,以及13^N-NH3·H2O GPET和18^F-FDG GPET测得左心室舒张末期容积(LVEDV)、左心室收缩末期容积(LVESV)和LVEF的差异;分析3种方法测得LVEF的相关性和一致性。结果心脏超声与13^N-NH3·H2O GPET、18^F-FDG GPET测得的LVEF值相关性和一致性较好(r=0.66、0.64,P<0.001);13^N-NH3·H2O GPET与18^F-FDG GPET测得的LVEF、LVEDV、LVESV的相关性和一致性较好(r=0.85、0.85、0.87,P<0.001)。结论对于病情严重的心肌梗死患者,心脏超声、13^N-NH3·H2O GPET、18^F-FDG GPET测得的LVEF值可以相互验证,13^N-NH3·H2O GPET和18^F-FDG GPET之间的左心室心功能参数具有非常好的相关性和一致性,均能较好地评价左心室功能。
Purpose To compare the difference of left ventricular ejection fraction(LVEF)among patients with severe myocardial infarction(MI)by echocardiography,13^N-NH3·H2O and 18^F-FDG GPET,and to study the correlation and consistency of values of left ventricular function measured by the last two methods after MI.Materials and Methods Clinical data of fifty patients with MI were retrospectively analyzed.Myocardial viability assessment with GPET and echocardiography were performed before percutaneous coronary intervention.The LVEF measured by three methods as well as LVEDV,LVESV and LVEF measured by the last two methods were respectively compared.In addition,the correlation of LVEF measured by the above three methods was analyzed.Results The LVEF measured by the three methods were well correlated and consistent(r=0.66,0.64,P<0.001).The LVEDV,LVESV and LVEF measured by the last two methods were well correlated and consistent(r=0.85,0.85,0.87,P<0.001).Conclusion The LVEF for patients with severe MI can be verified by echocardiography,13^N-NH3·H2O and 18^F-FDG GPET.The left ventricular function parameters are well correlated and consistent through the last two methods,which could be used to assess the left ventricular function more accurately.
