经皮微创置钉和开放椎弓根螺钉内固定术治疗无神经损伤胸腰椎骨折的疗效分析 被引量:34

摘要 目的探讨经皮微创置钉和开放椎弓根螺钉内固定术治疗无痛神经损伤胸腰段骨折的临床效果。方法采用随机数字表法将94例无神经损伤胸腰段骨折患者分为两组,开放手术组47例给予后路开放椎弓根螺钉内固定术治疗,微创组47例给予经皮微创椎弓根螺钉内固定术治疗,观察两组手术情况和临床疗效。结果至研究结束时,微创组脱落4例,剔除,纳入统计43例;开放手术组脱落6例,剔除,纳入统计41例。微创组手术时间、术中出血量、住院时间与开放手术组相比较均显著降低,差异均有统计学意义(P<0.05)。两组术前、术后6个月、术后2年椎体前缘高度比及后凸Cobb角相比较,差异均无统计学意义(P>0.05)。微创组术后第1、3、5天和6个月、2年的VAS评分与开放手术组相比较均显著降低,差异均有统计学意义(P<0.05)。结论经皮微创置钉治疗无神经损伤胸腰椎骨折与开放手术具有同样的手术效果,但可缩短手术时间,减少手术损伤,缩短住院时间,降低围手术期和远期腰部疼痛,可作为无神经损伤胸腰椎骨折的首选手术方式。
