视感知觉训练对不同类型和程度弱视儿童的疗效观察

Therapeutic effect of vi sual perception training on children with different types and degree of amblyopia

目的观察视感知觉训练对不同类型和程度弱视儿童的治疗效果。方法纳入2016年1月—2018年1月,唐山市眼科医院收治的弱视患儿400例(563只眼),根据治疗方法的不同分为观察组和对照组,观察组在配戴矫正眼镜基础上应用视感知觉训练疗法进行治疗,对照组在配戴矫正眼镜基础上应用遮盖法治疗。观察2组中不同治疗方法对不同类型(屈光不正、屈光参差和斜视)和不同程度(轻度、中度和重度)的弱视患儿的治疗效果。结果治疗6个月后,观察组的治疗总有效率(84.01%)高于对照组(71.75%),差异有统计学意义(x^2=12.405,P=0.002)。2种治疗方法对屈光不正性弱视,治疗效果相当,比较无统计学意义(P>0.05)。对于屈光参差性弱视,观察组治疗效果(85.71%)与对照组(70.33%)比较,差异有统计学意义(P<0.05)。对于斜视性弱视,观察组治疗效果(77.11%)与对照组(50.00%)比较,差异有统计学意义(P<0.05)。对于不同程度弱视,观察组对轻度和中度有效率优于对照组,差异均有统计学意义(P<0.05)。而对重度弱视,2组有效率差异无统计学意义(P>0.05)。结论视感知觉训练疗法治疗弱视的疗效总体优于遮盖法,对屈光参差和斜视类型以及轻中度的弱视患儿具有更好的治疗效果。

OBJECTIVE To observe the therapeutic effect of visual perception training on children with different types and degree of amblyopia.METHODS From January 2016 to January 2018,400 children(563 eyes)with amblyopia admitted to Tangshan Ophthalmological Hospital,were divided into observation group and control group according to different treatment methods.The cases of observation group were treated with visual perception training therapy on the basis of wearing correcting glasses,while the control group were treated with occlusion method on the basis of wearing correcting glasses.The therapeutic effects of different covering methods in the two groups on children with different types(ametropia,anisometropia and strabismus)and different degree(mild,moderate,and severe)were observed.RESULTS After 6 months of treatment,the total effective rate in the observation group(84.01%)was significantly higher than in the control group(71.75%),and the difference was statistically significant(x^2=12.405,P=0.002).There was no significant difference in the treatment effect of two treatment methods for ametropic amblyopia(P>0.05).For anisometropia amblyopia,the therapeutic effect of the observation group(85.71%)was better than the control group(70.33%),and the difference was statistically significant(P<0.05).For strabismus amblyopia,the therapeutic effect of the observation group(77.11%)was better than that of the control group(50.00%),the difference was statistically significant(P<0.05).For the different degree of amblyopia,the effective rate of the observation group was superior to the control group,and the difference was statistically significant(P<0.05).The effective difference of the two groups was not statistically significant(P>0.05).CONCLUSIONS The curative effect of visual perception training therapy in the treatment of amblyopia was better than that of covering method,and it had better therapeutic effect on anisometropia and strabismus type and mild to moderate amblyopia children.
