参柏合剂联合亚甲蓝治疗慢性肛周湿疹的疗效观察

Effect of Shenbo mixture combined with methylene blue for chronic perianal eczema

目的探讨参柏合剂熏洗坐浴联合亚甲蓝配比液局部封闭注射治疗慢性肛周湿疹的临床疗效。方法选取2017年1月至2018年1月在本院收治的120例慢性肛周湿疹患者为研究对象,将其随机分为观察组(n=60)与对照组(n=60)。两组患者均使用亚甲蓝配比液肛周点状封闭注射治疗,观察组在此基础上加用自拟中药复方参柏合剂熏洗坐浴,每天2次,早晚各1次,连续治疗2周(1个疗程),对照组患者同时应用等量相近温度的水熏洗坐浴。比较两组治疗前后瘙痒程度、皮损面积、皮损形态评分,临床疗效、半年复发情况,以及用药安全性。结果治疗前,两组瘙痒程度、皮损面积、皮损形态评分比较差异均无统计学意义(均P> 0.05);治疗后,两组上述各项评分低于治疗前,观察组评分低于对照组,差异均有统计学意义(均P <0.05)。观察组总有效率高于对照组,半年复发率低于对照组,差异均有统计学意义(均P <0.05)。两组患者均未出现影响治疗的明显不良反应。结论参柏合剂熏洗坐浴联合亚甲蓝配比液局部封闭注射治疗慢性肛周湿疹疗效确切,安全性良好,复发率低。

Objectives To investigate the effect of Shenbo mixture fumigation and sitz bath combined with local injection ofmethylene blue proportioning liquid in treating chronic perianal eczema. Methods One hundred and twenty patients with chron-ic perianal eczema treated in our hospital between January 2017 and January 2018 were recruited and randomly assigned to treat-ment group(n = 60) and control group(n = 60). Both groups received perianal point blocking injection with methylene blue pro-portioning liquid. Treatment group received, in addition, in-house Shenbo mixture fumigation and sitz bath twice per day(oneeach in the morning and the evening). Treatment lasted for two weeks(a course of treatment). Control group received warm sitzbath with equal volume and temperature. Degree of itching, skin lesion area, skin lesion appearance, effectiveness, recurrencewithin 6 months, and safety were compared between the two groups. Results Degree of itching, skin lesion area, and skin le-sion appearance did not differ between groups before treatment(P > 0.05). After treatment, these scores decreased in bothgroups and were significantly lower in the treatment group than in the control group(P < 0.05). Overall effectiveness was signifi-cantly higher and recurrence rate within 6 months was significantly lower in the treatment group(P < 0.05). No severe adverseevent was recorded. Conclusion Shenbo mixture fumigation and sitz bath combined with local injection of methylene blue pro-portioning liquid was effective and safe in treating chronic perianal eczema. It was associated with lower recurrence rate.
