中药外敷联合穴位针灸的中医辨证施护对急性脑梗死护理干预的效果分析

Effect of Traditional Chinese Medicine Dialectical Nursing by Traditional Chinese Medicine External Treatment Associating with Acupuncture on Nerve Function in Acute Cerebral Infarction Patients

目的:观察包括中药外敷与穴位针灸的中医辨证施护对急性脑梗死患者的干预效果。方法:随机选取2017年1月~2017年11月在安徽中医药大学第二附属医院治疗的急性脑梗死患者110例,入院后随机分为对照组和观察组各55例。对照组采用常规护理,观察组在对照组基础上采用包括中药外敷与穴位针灸的中医辨证施护,比较两组患者的美国国立卫生研究院卒中量表(NIHSS)评分、改良RANKIN量表(MRS)评分以及日常生活能力量表(ADL)生活质量(ADL)评分,干预80天随访时采用FACT-G(V4.0)生活质量核心量表对患者生活质量进行测定。结果:入院时观察组与对照组NIHSS评分、MRS评分及ADL评分无显著差异(P>0.05),经过护理干预后观察组NIHSS评分明显降低,MRS评分ADL评分明显增高,并且观察组患者在生理、社会家庭、情感、功能以及整体健康评分与对照组的差异具有统计学意义(P<0.05)。结论:采用中药外敷与穴位针灸的中医辨证施护可有效改善急性脑梗死患者的脑神经功能与生活质量,具有一定的临床推广应用价值。

Objective: To explore the effect of traditional Chinese medicine(TCM) dialectical nursing for patients with acute cerebral infarction in restoration stage by TCM external treatment associating with acupuncture.Methods: A total of 110 acute cerebral infarction patients in restoration stage,got treatment in The Second Affiliated Hospital of Anhui University of Traditional Chinese Medicine from January 2017 to November 2017,were randomly divided into experimental group(n=55) the controlled group(n=55).The controlled group was given routine nursing care,and the experimental group was given the dialectical nursing intervention by TCM external treatment associating with acupuncture.The nerve function and activities of daily living among patients were evaluated by National Institute of Health Stroke Scale(NIHSS),Modified Rankin Scale(MRS) and Activity of Daily Living Scale(ADL).The FACT-G core quality of life scale(V4.0) was used for determination of patients’ quality of life after intervention 80 days.Results: There was no significant difference in NIHSS score,MRS score and ADL score between the experimental group and the controlled group before nursing measures(P > 0.05).The NIHSS score of the experimental group after nursing was significantly lower than that in the controlled group and the difference was statistically significant(P < 0.05).The MRS score and ADL score of the experimental group after intervention were significantly higher than the controlled group.Besides,the physical score,socially family score,emotional score,function score and overall health score of the experimental group after intervention were significantly higher than the controlled group and the difference was statistically significant(P < 0.05).Conclusion: The dialectical nursing by TCM external treatment associating with acupuncture can improve the recovery of brain function,quality of life for patients with acute cerebral infarction in restoration stage,this result has a certain clinical application value.
