艾拉莫德联合甲氨蝶呤对类风湿关节炎炎症指标和骨代谢的影响

Effects of Iguratimod Combined with Methotrexate on Inflammatory Indexes and Bone Metabolism in Rheumatoid Arthritis

目的探析艾拉莫德联合甲氨蝶呤治疗类风湿关节炎对炎症指标和骨代谢的影响。方法于该院2017年8月—2018年10月收治的类风湿关节炎患者中方便选出100例,随机分入两组,对照组患者给予甲氨蝶呤治疗,观察组给予甲氨蝶呤+艾拉莫德治疗,对比两组的治疗效果。结果治疗6个月后的炎症因子TNF-α、IL-6、CRP、COX-2、M-CSF浓度比较:观察组分别为(4.7±0.4)pg/mL、(2.2±0.2)pg/mL、(4.1±0.5)mg/L、(10.1±1.0)pg/mL、(782±101)pg/mL,对照组分别为(6.8±1.0)pg/mL、(3.7±0.3)pg/mL、(6.8±0.7)mg/L、(14.6±1.2)pg/mL、(992±114)pg/mL,观察组均低于对照组(P<0.05);治疗6个月后的骨代谢指标:其中N-MID、TPⅠNP、[25(OH)D]浓度比较,观察组分别为(23.1±1.4)ng/mL、(49.4±2.9)ng/mL、(21.8±1.5)ng/mL,对照组分别为(17.9±1.5)ng/mL、(37.9±3.1)ng/mL、(18.3±1.6)ng/mL,观察组均高于对照组(P<0.05);β-CTX水平比较观察组为(0.39±0.05)pg/mL,低于对照组的(0.68±0.08)pg/mL(t=21.736,P<0.05)。结论甲氨蝶呤联合艾拉莫德治疗类风湿关节炎有助于促进患者炎症反应的缓解,改善骨代谢,控制病情进展,值得推广。

Objective To explore the effect of Iguratimod combined with methotrexate on inflammatory indexes and bone metabolism in the treatment of rheumatoid arthritis.Methods 100 patients with rheumatoid arthritis admitted to the hospital from August 2017 to October 2018 were convenienty selected and randomly divided into two groups.Patients in the control group were treated with methotrexate,while patients in the observation group were treated with methotrexate+Iramod,and the therapeutic effects of the two groups were compared.Results Six months after treatment,the levels of inflammatory factors TNF-alpha,IL-6,CRP,COX-2,M-CSF were compared:the observation group was(4.7±0.4)pg/mL,(2.2±0.2)pg/mL,(4.1±0.5)mg/L,(10.1±1.0)pg/mL,(782±101)pg/mL,The control group was(6.8±1.0)pg/mL,(3.7±0.3)pg/mL,(6.8±0.7)mg/L,(14.6±1.2)pg/mL and(992±114)pg/mL,The observation group was lower than the control group(P<0.05).Bone metabolic indexes after 6 months of treatment:The concentrations of N-MID,TP I NP and[25(OH)D]in the observation group were(23.1±1.4)ng/mL,(49.4±2.9)ng/mL,(21.8±1.5)ng/mL,while those in the control group were(17.9±1.5)ng/mL,(37.9±3.1)ng/mL,(18.3±1.6)ng/mL,which were higher than those in the control group(P<0.05).The level of beta-CTX in the observation group was(0.39±0.05)pg/mL,lower than that in the control group(0.68±0.08)pg/mL(t=21.736,P<0.05).Conclusion Methotrexate combined with Iguratimod in the treatment of rheumatoid arthritis is helpful to relieve inflammatory reaction of patients,improve bone metabolism and control disease progression,which is worthy of promotion.
