CO2激光联合5-ALA光动力治疗尖锐湿疣及对Th1/Th2细胞因子水平的影响 被引量:13

CO2 laser combined with 5-ALA photodynamic therapy for condyloma acuminatum and its effect on Th1/Th2 cytokine levels

摘要 目的采用CO2激光与氨基乙酸乙酯光动力疗法(ALA-PDT)联合治疗尖锐湿疣(CA),了解其疗效、复发情况、不良反应等,比较外周血中Th1/Th2细胞因子水平。方法选取2017年12月至2019年6月兰州市第一人民医院皮肤性病门诊诊治的216例尖锐湿疣患者作为研究对象。将这216例患者随机分为联合治疗组和激光治疗组,每组各108例。联合治疗组患者应用CO2激光联合5-ALA光动力治疗,激光组患者仅使用CO2激光治疗。比较两组患者的总有效率、复发率和不良反应发生率,并在治疗后6个月测定外周血细胞因子Th1/Th2。结果两组患者的年龄和性别差异无统计学意义。联合治疗组患者与激光组患者相比,联合治疗组患者的痊愈率、显效率、有效率、总有效率均高于激光组患者,差异具有统计学意义(P<0.05);联合治疗组患者的复发率明显低于激光组患者,差异具有统计学意义(P<0.05);联合治疗组患者的不良反应发生率与激光组患者比较,差异无统计学意义(P>0.05);联合治疗组患者IL-2和IFN-γ水平显著高于激光组患者,差异具有统计学意义(P<0.05);而联合治疗组患者IL-4、10水平明显低于激光组患者,差异具有统计学意义(P<0.05)。结论 CO2激光联合5-ALA光动力疗法治疗尖锐湿疣不增加不良反应发生率,是安全有效的,值得临床推广。 Objective To study the combination of CO2 laser and photodynamic therapy(ALA-PDT) in the treatment of condyloma acuminatum(CA) its efficacy, recurrence and adverse reactions and to compare the level of Th1/Th2 cytokines in peripheral blood. Methods Total of 216 patients with condyloma acuminatum treated in the dermatology and venereal disease clinic of Lanzhou First People′s Hospital from December 2017 to June 2019 were selected as the research subjects. These 216 patients were randomly divided into a combination treatment group and a laser treatment group, with 108 cases in each group. Patients in the combined treatment group were treated with CO2 laser combined with 5-ALA photodynamic therapy. Patients in the laser group were treated with CO2 laser only. The total effective rate, relapse rate and incidence of adverse reactions were compared between the two groups of patients, and the peripheral blood cytokines Th1/Th2 were measured 6 months after treatment. Results There was no significant difference in age and gender between the two groups. Compared with the patients in the laser group, the patients in the combination treatment group had higher recovery rates, marked efficiency, effective rates, and total effective rates than the patients in the laser group, and the differences were statistically significant(P<0.05). The recurrence rate was significantly lower than that in the laser group, and the difference was statistically significant(P<0.05). The incidence of adverse reactions in the combined treatment group was not statistically significant compared with the laser group(P>0.05). The levels of IL-2 and IFN-γ in patients were significantly higher than those in the laser group, and the difference was statistically significant(P<0.05). The levels of IL-4 and 10 in the combined treatment group were significantly lower than those in the laser group, and the difference was statistically significant(P<0.05).Conclusions CO2 laser combined with 5-ALA photodynamic therapy is safe and effective for the treatment of CA, which does not increase the incidence of adverse reactions and worthy of clinical promotion.
