益气活血化痰方对老年下肢动脉粥样硬化患者血脂及步态的影响研究

Effect of Yiqihuoxuehuatan Formula on Blood Lipid And Gait in the Elderly with Lower Extremity Atherosclerosis Disease

目的观察益气活血化痰方对老年下肢动脉粥样硬化(LEAD)患者血脂(TG、TC、LDL-C、HDL-C)、载脂蛋白E(ApoE)、步态时空参数(步频、步态周期、步幅、步速、左右步长、步长偏差)的影响。方法利用随机数字法将72例满足标准的患者随机分成2组,每组各36例。观察组予益气活血化痰方加阿托伐他汀钙片,20 mg每晚1次;对照组仅予阿托伐他汀钙片,治疗随访12周。观察治疗前后2组的血脂、载脂蛋白E水平及步态时空参数的变化。结果干预后观察组LDL-C降低(P<0.01),ApoE升高(P<0.01);对照组LDL-C有下降趋势,ApoE有升高趋势。治疗后观察组与治疗前相比,步幅、步速、左右步长增加,步长偏差低于治疗前,差异有统计学意义(P<0.01);观察组疗效优于对照组。结论益气活血化痰方合用阿托伐他汀可有效降低老年LEAD患者的LDL-C水平,对改善TG、ApoE水平及步态有益。

Objectives To observe the effect of Yiqihuoxuehuatan Formula on blood lipid indices(TG,TC,LDL-C and HDL-C),apolipoprotein E(ApoE)level and gait parameters(cadence,gait cycle,stride length,Velocity,step length,step deviation)in the elderly with lower extremity atherosclerotic disease(LEAD)Methods 72 cases with LEAD were randomly divided into 2 groups:treatment group and control group,36 in each;treatment with Yiqihuoxuehuatan Formula plus atorvastatin calcium tablets(20 mg per night)was given to cases in treatment group while treatment with only atorvastatin calcium tablets to cases in control group;the course lasted for 12 weeks in both groups;the levels of serum TC,TG,HDL-C and LDL-C,ApoE and the gait parameters of all the cases before and after treatment were observed.Results After intervention,the LDL-C level decreased significantly(P<0.01)ang the ApoE level increased significantly(P<0.01)in treatment group while the level of LDL-C showed a downward trend and the level of ApoE a upward trend in control group;after treatment,the stride,step speed and both left and right striding length were greater while the step deviation was smaller than those before treatment in treatment group(P<0.01);the curative effect in treatment group was superior to that in control group.Conclusion Yiqihuoxuehuatan Formula plus atorvastatin calcium tablets can effectively decrease the level of LDL-C,optimize the levels of TG and ApoE and improve gait parameters in the elderly with LEAD.
