乳腺钼靶联合MRI检查在中老年乳腺癌诊断中的应用价值

Application value of mammography combined with MRI in diagnosis of breast cancer in middle-aged and elderly patients

目的探讨乳腺钼靶联合MRI检查在中老年乳腺癌诊断中的价值。方法选取2017年3月~2018年8月本院收治的60例中老年乳腺肿瘤患者作为研究对象,所有患者均行乳腺钼靶及MRI检查,以病理检查作为金标准。比较乳腺钼靶、MRI检查及乳腺钼靶联合MRI检查对乳腺癌检出的灵敏度、特异度、准确率。结果乳腺钼靶诊断的灵敏度、特异度、准确率分别为56.25%、53.57%、55.00%;MRI诊断的灵敏度、特异度、准确率分别为62.50%、67.86%、65.00%;乳腺钼靶联合MRI检查的灵敏度、特异度、准确率分别为84.38%、92.86%、88.33%。乳腺钼靶联合MRI检查对乳腺癌检出的灵敏度、特异度、准确率均高于单独检查,差异均有统计学意义(P<0.05);乳腺钼靶与MRI单独检查的灵敏度、特异度、准确率比较,差异无统计学意义(P>0.05)。结论相较于乳腺钼靶及MRI单独检查,乳腺钼靶联合MRI检查对中老年乳腺癌的检出能力更强,诊断准确率、灵敏度和特异度更高,值得临床应用推广。

Objective To explore the value of mammography combined with MRI in the diagnosis of breast cancer in middle-aged and elderly people.Methods From March 2017 to August 2018,60 cases of middle-aged and elderly breast tumor patients admitted to our hospital were selected as research objects.All patients underwent mammography and MRI examination,with pathological examination as the gold standard.To compare the sensitivity,specificity and accuracy of mammography,MRI and mammography combined MRI in breast cancer detection.Results The sensitivity,specificity and accuracy of mammography were 56.25%,53.57%and 55.00%respectively.The sensitivity,specificity and accuracy of MRI diagnosis were 62.50%,67.86%and 65.00%respectively.The sensitivity,specificity and accuracy of mammography combined with MRI were 84.38%,92.86%and 88.33%respectively.The sensitivity,specificity and accuracy of mammography combined with MRI in breast cancer detection were higher than those of single examination,and the differences were statistically significant(P<0.05).There were no significant difference in sensitivity,specificity and accuracy between mammography and MRI(P>0.05).Conclusion Compared with mammography and MRI alone,mammography combined with MRI has stronger detection ability,higher diagnostic accuracy,sensitivity and specificity for middle-aged and elderly breast cancer,which is worthy of clinical application and promotion.
