剖宫产产妇产褥感染危险因素调查及护理对策

Investigation on Risk Factors of Puerperal Infection in Cesarean Section and its Nursing Countermeasures

目的探讨剖宫产产妇产褥感染危险因素及护理对策。方法选取2015年1月~2017年12月我院收治的剖宫产产妇99例,通过回顾性分析收集产妇临床资料,统计产褥感染产妇例数,经单因素、多因素Logistic回归分析明确产褥感染危险因素,并确定相应护理对策。结果产褥感染9例,感染发生率为9.09%;单因素分析结果显示,手术时间、术后出血、妊娠期糖尿病、妊娠期生殖道感染、胎膜早破、血红蛋白水平、瘢痕子宫是剖宫产产褥感染的相关因素(P<0.001,P<0.05)。多因素Logistic回归分析结果显示,术后出血、妊娠期糖尿病、妊娠期生殖道感染、胎膜早坡、血红蛋白<40g/L、瘢痕子宫是剖宫产产妇产褥感染的危险因素(P<0.001)。结论我院剖宫产产妇产褥感染发生率为9.09%。术后出血、妊娠期糖尿病、妊娠期生殖道感染、胎膜早破、血红蛋白<40g/L、瘢痕子宫是剖宫产产妇产褥感染危险因素,故需加强产褥感染预警防控,积极开展产褥感染风险健康教育,全面完善临床护理工作。

Objective To explore the risk factors and nursing strategies for puerperal infection in cesarean section.Methods 99 cases of cesarean section women admitted to our hospital from January 2015 to December 2017 were selected.The clinical data of maternal collection were collected by retrospective analysis.The number of maternal infections was counted by single factor and multivariate logistic regression analysis.Identify the risk factors for puerperal infection and determine the appropriate care strategies.Results According to the survey,the number of cases of puerperal delivery in our hospital was 9 cases,and the infection rate was 9.09%.The single factor analysis showed that the factors related to puerperal maternal puerperal infection were operative time,postpartum hemorrhage,pregnancy.Stage diabetes,genital tract infection during pregnancy,premature rupture of membranes,hemoglobin level,scar uterus(P<0.001,P<0.05);multivariate logistic regression analysis showed that the risk factors for puerperal maternal puerperal infection were postpartum hemorrhage,gestational diabetes,Genital tract infection during pregnancy,premature rupture of membranes,hemoglobin<40g/L,scar uterus(P<0.001).Conclusion The incidence of puerperal infection in cesarean section in our hospital is 9.09%.The operation time≥1.5h,postpartum hemorrhage,gestational diabetes,genital tract infection during pregnancy,premature rupture of membranes,low hemoglobin and scar uterus are the risk factors for maternal puerperal infection.It should strengthen the prevention and control of puerperal infection.
