经筋灸法治疗腰背肌筋膜炎随机对照研究

A Randomized Controlled Trials Of The Curative Effect Of Tendons Moxibustion Of Lumbar Back Muscle Fasciitis

目的:观察艾灸结筋病灶点与传统经穴治疗腰背肌筋膜炎的疗效差异。方法:将78例腰背肌筋膜炎患者,随机分为经筋组(38例)和对照组(40例)。经筋组患者先进行足太阳经筋触诊,确定结筋病灶点部位后按照艾灸的流程施治,常见结筋病灶点为环跳次、志室次、肓门次、腰椎横突等;对照组根据《针灸推拿学》选择腰眼、阿是穴、大肠俞、委中辨证取穴进行艾灸治疗。两组均每2d1次,共治疗8周。观察记录0周、2周、4周、8周两组患者的VAS疼痛评分、Roland-Morris功能评分,腰部静、动态肌耐力评估,并进行疗效评价。结果:①经筋组疗效明显优于对照组(P<0.05);②不同时间点之间VAS、RMDQ、腰部静动态肌耐力比较,自第2疗程开始两组之间差异有统计学意义(P均<0.05);③时间因素与治疗方案之间存在交互作用(P均<0.05),说明时间因素发挥的作用在两种方案上的大小有差异,经筋组随着治疗疗程时间的延长,评分改善的效果显著高于对照组。结论:基于经筋理论艾灸治疗腰背肌筋膜炎可以有效缓解疼痛,改善腰部功能,且持续治疗时间超过4周后疗效显著。

Objective:To observe the curative effect of tendons moxibustion and traditional moxibustion on the treatment of low back muscle fasciitis.Methods:78 patients were randomly divided into two groups which is the tendon group(38 cases)and the control group(40 cases).Patients in the tendon group were firstly treated with palpation of the foot,and the sites of the focals were determined according to the procedure of acupuncture.The common focals were Huantiaoci,Zhishici,Huangmenci,Yaozhuihengtu,etal.Both groups were treated once during 2 days for 8 weeks.The VAS,Roland-Morris dysfunction questionnaire,lumbar static and dynamic muscle endurance assessment and efficacy evaluation were performed at 0,2,4,and 8 weeks.Results:(1)The efficacy of the tendon group was better than that of the control group(P<0.05);(2)VAS,RMDQ,and lumbar static dynamic muscle endurance at different time points were statistically significant between the two groups(P<0.05).(3)There was an interaction between the time factor and the treatment plan(P<0.05),indicating that the effect of the time factor was different in the size of the two regimens,and the length of the treatment group was prolonged with the treatment course,the score improvement effect is significantly higher than the control group.Conclusion:The Tendons Moxibustion treatment of lumbar back muscle fasciitis can effectively relieve pain and improve lumbar function,and the effect is significant after continuous treatment for more than 4 weeks.
