重庆市荣昌区经异性性行为感染的≥50岁男性HIV/AIDS病人空间自相关分析

Spatial autocorrelation analysis of HIV/AIDS male patients aged 50 years or above with heterosexual infection in Rongchang district, Chongqing

目的了解2006-2019年重庆市荣昌区经异性性行为感染的≥50岁男性艾滋病病毒(HIV)感染者/艾滋病(AIDS)病人(简称HIV/AIDS病人)的空间分布特征,为制定相应的艾滋病防控策略提供科学依据。方法采用GeoDA 0.95i软件对报告率进行空间经验贝叶斯平滑和空间自相关分析,采用地理信息系统(GIS)实现结果的可视化展示。结果2006-2019年,荣昌区共计报告经异性行为性感染的≥50岁男性HIV/AIDS病人843例。2006-2012年、2013-2019年、2006-2019年累计报告率全局Moran’sI值分别为0.3027、05499、0.5551(P均<0.05,Z均>1.96),显示荣昌区经异性性行为感染的≥50岁男性HIV/AIDS病人累计报告率整体空间分布表现为空间聚集性,且聚集特征逐年明显。空间经验贝叶斯平滑后的累计报告率等级地图和LISA聚集地图均显示,昌州街道、峰高街道、万灵镇、直升镇、河包镇、古昌镇为高发聚集地区。结论应重点加强中老年人群警示性健康教育,进一步通过对该区域中老年男性既往商业性行为信息抽样调查研究等措施来挖掘隐匿的低档场所暗娼,精准防控,避免不安全商业性行为感染艾滋病,全面有效控制荣昌区中老年人群艾滋病的流行。

Objective To understand the spatial distribution characteristics of heterosexually infected HIV/AIDS male patients aged 50 years or above in Rongchang district,Chongqing from 2006 to 2019,and provide a scientific basis for developing the HIV prevention and control strategies.Methods GeoDA 0.95 i software was used to carry out spatial empirical bayesian smoothing and spatial auto-correlation analysis of the report rate,and geographic information system(GIS)was used to visualize the results.Results From 2006 to 2019,a total of 843 cases of heterosexually infected HIV/AIDS male patients aged 50 years or above were reported.The global Moran’S I values for the cumulative report rates of 2006-2012,2013-2019 and 2006-2019 were 0.3027,05499 and 0.5551,respectively(all P<0.05,all Z>1.96),indicating that the overall spatial distribution of the cumulative report rate of heterosexually infected males was spatially clustered and the clustering characteristics were evident year by year.The graded map after spatial empirical bayesian smoothing and LISA cluster map showed that the streets of Changzhou,Fenggaot,and the towns of Wanling,Zhisheng,and Hebao were the high-accumulation areas.Conclusion It is important to strengthen the health education of middle-aged and elderly people in the areas of Changzhou,Fenggao,Wanling,Zhisheng,Hebao,and Guchang in Rongchang district.Furthermore,a sample survey is suggested to conduct on the commercial sexual behavior of the middle-aged and elderly in the areas to access to the hidden low-fee sex workers for AIDS prevention and control,esp.in the group of the middle-aged and elderly in Rongchang district.
