氟康唑联合碳酸氢钠治疗鼻咽癌放射性口腔黏膜炎真菌感染疗效 被引量:15

Effect of fluconazole combined with sodium bicarbonate on treatment of nasopharyngeal carcinoma radioactive oral mucositis patients with fungal infection

摘要 目的探究氟康唑联合碳酸氢钠对鼻咽癌放射性口腔黏膜炎真菌感染患者口腔白斑面积及口腔pH的疗效评价。方法选取于2018年2月-2019年2月在山东省滨州医学院附属医院进行诊治的鼻咽癌放射性口腔黏膜炎真菌感染患者88例,利用随机数字表法分为对照组(44例)和研究组(44例),对照组采用氟康唑进行治疗,研究组采用氟康唑联合碳酸氢钠治疗。观察治疗后患者口腔白斑面积、pH值变化及治疗有效率,同时记录不良反应发生率。结果在治疗2周后,对照组治愈18例、好转15例,总有效率为77.27%;研究组治愈23例、好转18例,总有效率为93.18%,高于对照组(P<0.05)。经过不同方案治疗后,两组患者口腔的白斑面积及pH均有改善(P<0.05)。治疗后,研究组口腔白斑面积为(2.65±1.06) mm^2,小于对照组(7.46±1.94) mm^2(P<0.05);研究组口腔pH值为7.45±0.76,高于对照组6.24±0.74(P<0.05),且两组患者的不良反应发生率无统计学差异。结论氟康唑联合碳酸氢钠对鼻咽癌放射性口腔黏膜炎相关真菌感染具有良好的临床疗效,药物的联合应用能够显著降低口腔内白斑面积,改善口腔环境,且安全性较高。 OBJECTIVE To explore the effect of fluconazole combined with sodium bicarbonate on oral leukoplakia and oral pH of the nasopharyngeal carcinoma patients with radioactive oral mucositis.METHODS A total of 88 nasopharyngeal carcinoma patients with radioactive oral mucositis fungal infection who were treated in the Affiliated Hospital of Binzhou Medical College from Feb 2018 to Feb 2019 were enrolled in the study and randomly divided into the control group with 44 cases and the study group with 44 cases.The control group was treated with fluconazole,and the study group was given fluconazole combined with sodium bicarbonate.The oral leukoplakia area,oral pH value and effective rate of treatment of the patients were observed after the treatment,and the incidence rates of adverse reactions were recorded.RESULTS After the treatment for 2 weeks,18 patients were cured in the control group,and 15 patients were improved;23 patients were cured in the study group,and 18 patients were improved;the total effective rate of the study group was 93.18%,significantly higher than 77.27%of the control group(P<0.05).The oral leukoplakia area and oral pH of the two groups of patients were significantly improved after different treatment programs were carried out(P<0.05).The oral leukoplakia area of the study group was(2.65±1.06)mm^2 after the treatment,significantly less than(7.46±1.94)mm^2 of the control group(P<0.05).The oral pH values of the study group was 7.45±0.76,significantly higher than 6.24±0.74 of the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups of patients.CONCLUSION Fluconazole combined with sodium bicarbonate can achieve significantly effect on treatment of the nasopharyngeal carcinoma patients with radioactive oral mucositis,and the combined use of drugs can significantly reduce the oral leukoplakia area and improve the oral environment,with the safety high.
