带绊纽扣钢板治疗RockwoodⅢ型肩锁关节脱位的临床观察

Effect of the treatment for RockwoodⅢtype acromioclavicular joint dislocation with a trip button plate

目的分析带绊纽扣钢板治疗RockwoodⅢ型肩锁关节脱位的临床效果。方法2018年1月至2019年1月重庆市渝北区人民医院骨一科收治的42例RockwoodⅢ型肩锁关节脱位患者,数字表法随机分为观察组和对照组,每组21例。对照组行常规锁骨钩钢板固定治疗;观察组行带绊纽扣钢板内固定治疗。对比两组患者治疗有效率、肩关节功能Constant-Murley评分、并发症发生率。结果观察组患者治疗有效率为95.24%(20/21),并发症发生率为14.29%(3/21);对照组患者治疗有效率为80.95%(17/21),并发症发生率为28.57%(6/21);两组对比,差异无统计学意义(P>0.05)。观察组术后1周、术后3个月的Constant-Murley评分与对照组相比差异有统计学意义(66.2±7.4)、(80.8±8.6)分比(55.6±8.2)、(67.4±9.7)分,(P<0.05)。结论带绊纽扣钢板治疗RockwoodⅢ型肩锁关节脱位肩关节功能恢复良好。

Objective To analyze the clinical effect of treatment for RockwoodⅢtype acromioclavicular joint dislocation with a trip button plate.Methods From January 2018 to January 2019,42 patients with RockwoodⅢtype acromioclavicular joint dislocation admitted to Department of Orthopaedics,Yubei District People's Hospital,were randomly divided into observation group and control group,21 cases in each group.The control group was treated with conventional clavicular hook plate fixation.The observation group was treated with trip button plate fixation.The treatment effectiveness,shoulder-function Constant-Murley score,and complication rate compared.Results The effective rate of the observation group was 95.24%(20/21);complication rate was 14.29%(3/21).The effective rate of the control group was 80.95%(17/21);complication rate was 28.57%(6/21).The difference between the two groups was not statistically significant(P>0.05).The Constant-Murley score of the observation group at 1 week and 3 months after surgery was(66.2±7.4)points and(80.8±8.6)points,while in the control group was(55.6±8.2)points and(67.4±9.7)points;the difference was significant(P<0.05).Conclusion Treatment of RockwoodⅢtype acromioclavicular joint dislocation with shackle button steel plate is good for shoulder function recovery.
