摘要
目的探讨急性左心衰竭患者在急诊急救中使用无创呼吸机的治疗效果。方法选取山东医学高等专科学校附属医院2018年5月至2019年5月收治的60例急性左心衰竭患者作为研究对象,按照随机数字表法分为对照组(30例)与观察组(30例)。对照组采取传统治疗方法,观察组采用无创呼吸机治疗。对比两组患者的治疗有效率、心脏事件、再住院率、血氧饱和度(SpO2)、静息心率(HR)、二氧化碳分压(PaCO2)等指标。结果治疗后,观察组中显效15例(50.00%),有效11例(36.67%),无效4例(13.33%),治疗总有效26例(86.67%);对照组中显效10例(33.33%),有效11例(36.67%),无效9例(30.00%),治疗总有效21例(70.00%),组间比较差异有统计学意义(P<0.05)。观察组中发生心脏事件1例(3.33%)、再次住院7例(23.33%),对照组中发生心脏事件5例(16.67%)、再次住院15例(50.00%),组间比较差异有统计学意义(P<0.05)。观察组患者治疗后PaCO2为(87.6±11.5)mm Hg,SpO2为(36.8±3.1)mm Hg,高于对照组的(78.7±12.2)mm Hg和(35.1±4.3)mm Hg,组间比较差异有统计学意义(P<0.05)。观察组的HR为(80.2±8.1)次/min,明显低于对照组的(86.2±7.0)次/min,组间比较差异有统计学意义(P<0.05)。结论针对急性左心衰竭患者采用无创呼吸机治疗的临床效果较好,且安全性高。
Objective To explore the application effect of non-invasive ventilator in emergency emergency for acute left heart failure.Methods A total of 60 patients with acute left heart failure admitted to the Affiliated Hospital of Shandong Medical College from May 2018 to May 2019 were selected as study subjects and divided into control group and observation group according to the random number table method,with 30 patients in each group.The control group was treated with traditional therapy,while the observation group was treated with non-invasive ventilator.The treatment response rate,cardiac events,rehospitalization rate,oxygen saturation(SpO2),resting heart rate(HR),and partial pressure of carbon dioxide(PaCO2)were compared between the two groups.Results The observation group was 15 case(50.00%),effective 11 case(36.67%),ineffective 4 case(13.33%),the total effective rate of observation group was 26 case(86.67%),the control group was 10 case(33.33%),effective 11 case(36.67%),ineffective 9 case(30.00%),and the total effective rate of the control group was 21 case(70.00%),and the observation group was significantly higher than the control group(P<0.05).The cardiac events 1 case(3.33%),the re hospitalization rate 7 case(23.33%)in the observation group were lower than that of the control group(16.67%),and the re hospitalization rate was 15 case(50.00%),the difference was statistically significant(P<0.05).The PaO2(87.6±11.5)mm Hg,SpO2(36.8±3.1)mm Hg in the observation group were higher than that of the control group(78.7±12.2)mm Hg,SpO2(35.1±4.3)mm Hg,and HR(80.2±8.1)times/min of the observation group was significantly lower than that of the control group(86.2±7.0)times/min,and the difference was statistically significant(P<0.05).Conclusion The noninvasive ventilator is effective and safe in the treatment of acute left heart failure.
