经阴道超声诊断剖宫产切口瘢痕妊娠的价值

Value of Transvaginal Ultrasound in Diagnosis of Scar Pregnancy in Cesarean Section

目的探讨经阴道彩色多普勒超声诊断剖宫产切口瘢痕妊娠的临床价值。方法选取在2018年7月—2019年10月在本院进行彩色多普勒超声诊断剖宫产切口瘢痕妊娠的80例患者进行研究。根据随机分配的原则将80例患者分为观察组和对照组。对照组患者采用经腹部彩超的诊断方式,观察组患者采用经阴道彩色多普勒超声诊断。通过对比两组患者的诊断正确率判断经阴道彩色多普勒超声诊断剖宫产切口瘢痕妊娠的临床价值。结果对比观察组和对照组的诊断正确率,剖宫产切口瘢痕妊娠的诊断中应用经阴道彩色多普勒超声检查优于腹部超声多普勒超声检查,即观察组患者的诊断结果正确率优于对照组(从两组患者超声检查准确率对比来看,观察组患者的确诊率为95%,对照组患者的确诊率为75%);通过对比瘢痕妊娠治疗前后超声检查的结果,治疗前后的病灶径长略微长于治疗后,差异无统计学意义(P>0.05),即t=0.701,P=0.485;治疗前病灶径宽于治疗后,治疗后的子宫前壁肌层厚度和动脉血流阻力指数均高于治疗前,差异有统计学意义(P<0.05),即病灶径宽对比t=2.233,P=0.028,子宫前壁肌层厚度对比t=9.687,P=0.000,动脉血流阻力指数t=7.017,P=0.000。从整体上判断,差异有统计学意义(P<0.05)。结论经阴道彩色多普勒超声诊断剖宫产切口瘢痕妊娠具有很高的临床应用价值,提高诊断效率及医院的绩效。

Objective To probe into the clinical value of transvaginal color Doppler ultrasound in diagnosis of scar pregnancy in cesarean section.Methods 80 cases of patients with cesarean section pregnancy and who diagnosed by color Doppler ultrasound in the hospital from July 2018 to October 2019 were selected for research,and the patients were divided into observation group and control group according to the principle of random distribution.The control group was diagnosed by transabdominal color Doppler ultrasound,and the observation group was diagnosed by color Doppler ultrasound.Then,by comparing the diagnostic accuracy of the two groups of patients,the clinical value of transvaginal color Doppler ultrasound in the diagnosis of cesarean incision scar pregnancy was judged.Results The diagnostic accuracy of the observation group and the control group were compared,and the application of transvaginal color Doppler ultrasound in diagnosis of cesarean scar pregnancy was better than that in the abdominal ultrasonography.The diagnostic accuracy in the observation group was better than that in the control group(the accuracy of ultrasound examination between the two groups were compared,the diagnostic accuracy in the observation group was 95%,and in the control group was 75%).The results of ultrasound before and after the treatment of scar pregnancy were compared.The diameter of lesions before and after treatment was slightly longer than that after treatment,and there was no statistically significant difference(P>0.05),t=0.701,P=0.485.The lesion diameter before treatment was wider than after treatment.After treatment,the myometrial thickness of the anterior wall of uterus and the arterial resistance index were significantly higher than those before treatment,the difference was statistically significant(P<0.05),the diameter and width of the focus t=2.233,P=0.028,the thickness of myometrium in anterior wall of uterus t=9.687,P=0.000,the arterial resistance index t=7.017,P=0.000,from the whole,the difference was statistically significant(P<0.05).Conclusion The clinical application value of transvaginal color Doppler ultrasound in diagnosis of scar pregnancy in cesarean section are high,which can improve the efficiency of diagnosis and the performance of the hospital.
