内镜下精准贲门胃静脉曲张断流术的应用效果

Application effect of accurate cardiac gastric varicose devascularization under endoscope

目的探讨内镜下精准贲门胃静脉曲张断流术(ESVD)的应用效果。方法选取2018年5月~2019年1月九江学院附属医院诊治的肝硬化食管胃静脉曲张破裂出血患者100例作为研究对象,按随机数字表法分为观察组(n=50)和对照组(n=50)。观察组采用ESVD治疗,对照组采用传统治疗的方式,比较两组治疗有效性、再出血发生率及随访死亡情况。结果观察组治疗初次止血成功率、静脉曲张一次性消除有效率均高于对照组,手术操作时间短于对照组,差异有统计学意义(P<0.05)。观察组的住院时间短于对照组,再出血发生率、死亡率低于对照组,差异均有统计学意义(P<0.05)。结论ESVD能提高肝硬化门静脉高压食管胃静脉曲张破裂出血患者的治疗效果,改善患者预后,可作为其安全有效的内镜治疗方案。

Objective To investigate the application effect of accurate cardiac gastric varicose devascularization under endoscope(ESVD).Methods From May 2018 to January 2019,100 patients with ruptured esophageal and gastric varicose bleeding from liver cirrhosis treated in Affiliated Hospital of Jiujiang University were selected as subjects.They were divided into observation group(n=50)and control group(n=50)according to random number table method.The observation group was treated with ESVD,the control group was treated with the traditional way.The effectiveness of treatment,the incidence of rebleeding and the mortality of follow-up in the two groups were compared.Results The success rate of initial hemostasis and the effective rate of one-time elimination of varicose veins in the observation group were higher than those in the control group,and the operation time was shorter than that in the control group,and the differences were statistically significant(P<0.05).The hospital stay in the observation group was shorter than that in the control group,and the incidence of rebleeding and mortality were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion ESVD can improve the therapeutic effect and prognosis of patients with esophageal and gastric varicose bleeding due to portal hypertension in liver cirrhosis.It can be used as a safe and effective endoscopic treatment.
