盐阈值与外周动脉粥样硬化的相关性研究

摘要 目的探索盐阈值与外周动脉粥样硬化的相关性,为预防外周动脉粥样硬化发生、发展提供科学依据。方法采用横断面研究设计,选取2017年12月至2019年1月陆军军医大学大坪医院心血管内科住院的2079例患者为研究对象,应用梯度浓度的NaCl溶液测定患者的盐阈值(测试实施者遵循盲法),而后进行双侧颈动脉和双侧股动脉超声检查,根据是否存在外周动脉粥样斑块分为外周动脉粥样斑块组和无外周动脉粥样斑块组,比较两组盐阈值并分析盐阈值与外周动脉粥样硬化的关系。结果外周动脉粥样斑块组盐阈值明显高于无外周动脉粥样斑块组[(0.07±0.04)vs.(0.05±0.04),P<0.05]。多因素logistic回归模型在控制外周动脉粥样硬化的传统危险因素后,发现盐阈值不独立增加外周动脉粥样斑块发生的风险[(OR=0.919,95%CI:0.477~1.774),P=0.802]。结论外周动脉粥样斑块患者的盐阈值较高,但盐阈值升高不是发生外周动脉粥样硬化的独立危险因素。 Objective To explore the relationship between salt taste threshold and peripheral atherosclerosis,and provide scientific basis for prevention of peripheral atherosclerosis.Methods A cross-sectional study design was adopted to use NaCl solution of gradient concentration to measure the salt taste threshold of the patients(the test implementer followed a blind method),and conduct bilateral carotid and femoral arteries ultrasound examination in 2079 patients admitted to the Department of Cardiology,Daping Hospital from December 2017 to January 2019.According to the presence or absence of peripheral atherosclerotic plaque,the patients were divided into the peripheral atherosclerotic plaque group and the non-peripheral atherosclerotic plaque group.The difference of salt taste threshold between the two groups was compared with nonparametric test.The relationship between the salt taste threshold and peripheral atherosclerosis was analyzed.Results The salt taste threshold of the peripheral atherosclerotic plaque group was significantly higher than that of the non-peripheral atherosclerotic plaque group[(0.07±0.04)vs.(0.05±0.04),P<0.05].After multivariate logistic regression model controlled for traditional risk factors of peripheral atherosclerosis,salt taste threshold did not increase the risk of peripheral atherosclerotic plaque[(OR=0.919,95%CI:0.477-1.774),P=0.802].Conclusion The salt taste threshold was higher in patients with peripheral atherosclerotic plaque,but the increased salt taste threshold was not an independent risk factor for the development of peripheral atherosclerosis.
