摘要
目的:采用Meta分析的方法对国内双能计算机断层扫描(dual-energy computed tomography,DECT)诊断痛风的效能进行评价。方法:计算机检索Pubmed、Embase、Cnki、WanFang Date和CBM数据库,收集有关DECT对国内痛风患者诊断的相关文献。检索时间从建库到2020年9月,由2位研究者独立筛选文献、提取数据并评价纳入研究的偏倚风险,采用Stata 14.0软件进行Meta分析,计算合并灵敏度、特异度、阳性拟然比、阴性拟然比和诊断比值比以及95%CI,绘制SROC曲线,并计算曲线下面积,使用敏感性分析探索潜在的异质性来源。结果:共纳入22篇文献,汇总灵敏度、特异度、阳性拟然比、阴性拟然比和诊断比值比以及95%CI分别为0.93(0.91~0.95)、0.92(0.89~0.94)、11.8(8.47~16.43)、007(0.05~0.10)、158.22(98.90~261.04),SROC曲线下面积为0.97。结论:DECT对我国痛风患者有较高的诊断价值,但受纳入研究数量和质量的限制,需要更多高质量研究予以验证。
Objective To evaluate the diagnostic efficiency of dual-energy computed tomography(DECT)for gout in Chinese patients with the method of Meta analysis.Methods Pubmed,Embase,Cnki,WanFang Date and CBM database were searched on a computer.Relevant literatures about the diagnostic value of DECT for domestic gout patients were collected.The search time was from the establishment of the database to September 2020.Two researchers independently screened the literature,extracted data,and evaluated the risk of biasin the included studies.Stata 14.0 software was used for Meta analysis to calculate the combined sensitivity,specificity,positive likelihood ratio,negative likelihood ratio,diagnostic odds ratio and 95%CI,SROC curve was drawn,the area under the curve was calculated,and the potential source of heterogeneity was explored by sensitivity analysis.Results A total of 22 literatures were included.The sensitivity,specificity,positive likelihood ratio,negative likelihood ratio,diagnostic odds ratio and 95%CI were 0.93(0.91~0.95),0.92(0.89~0.94),11.8(8.47~16.43),0.07(0.05~0.10)and 158.22(98.90~261.04),respectively.The area under the SROC curve was 097.Conclusion DECT has high diagnostic value for Chinese patients with gout.Due to limited quality and quantity of the included studies,more high quality studies are required to verify the above conclusion.
