摘要
目的观察平原(海拔57 m)成年健康人群急进到高原地区(海拔4300m)后其皮质醇(Cortisol)水平的改变,以及发生急性高原反应(AMS)时,重度AMS与非重度AMS之间皮质醇水平差异比较,探讨皮质醇在应激下的分泌变化与AMS的关系。方法45名平原成年健康人,分别采集他们身处平原地区和高原地区的静脉血,使用罗氏电化学发光免疫法(ECLIA)检测血浆皮质醇水平。根据2018年新的《路易斯湖评分标准》,将急性高原反应病分为重度反应组和非重度反应组进行比较。结果与在平原地区比较,平原人群在急进高原后,血浆皮质醇水平显著升高,从305.01±60.42nmol/L升至416.30±84.68nmol/L(P<0.05)。AMS重度反应组皮质醇水平,由298.54±37.12nmol/L升到417.82±2840nmol/L,升高(39.94±6.14)%,AMS非重度反应组,由314.15±20.31nmol/L升到384.20±32.87nmol/L,升高(22.30±5.14)%。结论健康成人由平原急进到高原时,机体的皮质醇水平在短期内呈现出较大的波动,高原皮质醇水平高于平原,且AMS重度反应组升高百分比大于AMS非重度组。
Objective To observe the changes in the cortisol concentration of healthy adults in the plain region(57m above sea level)to the high-altitude region(4300m above sea level),and the cortex between the severe AMS group and non-severe acute AMS when acute altitude sickness(AMS)occurs and to compare the difference of alcohol concentration and to explore the relationship between cortisol secretion changes under stress and AMS.Methods Venous blood was collected at 8∶00 a.m.in plain and high-altitude regions,respectively,from 45 medical team members of the research subject.The concentration of plasma corticosteroids was measured by ECLIA.According to the Lewis Lake scoring standard,acute high-altitude sickness was divided into the severe reaction group and non-severe reaction group for a comparison.Results Compared with those subjects in plain areas,the plasma cortisol concentration of healthy people increased significantly from 305.01±60.42 nmol/L to 416.30±84.68 nmol/L(P<0.05).The cortisol concentration of AMS severe reaction group increased from 298.54±37.12 to 417.82±28.40 nmol/L,with an increasement of(39.94±6.14)%,while that of AMS non-severe reaction group increased from 314.15±20.31 to 384.20±32.87 nmol/L,with an increasement of(22.30±5.14)%.Conclusion When the healthy adults rush to the plateau area from the plain region,the cortisol concentration of the body shows a large fluctuation in a short period of time.The cortisol concentration in the plateau region is higher than that in the plain region,and the percentage of the increase in AMS severe reaction group is higher than that in AMS non-severe group.
