摘要
目的:检测结直肠癌(CRC)患者血清长链非编码RNA肺癌相关转录本1(LUCAT1)的水平,探讨其在CRC辅助诊断中的临床应用价值。方法:选取2017-10—2018-12病理诊断为CRC的患者51例,结直肠息肉患者43例,CRC术后患者24例,以健康体检者50名作为健康对照组。采用实时荧光定量法测定LUCAT1,化学发光法测定癌胚抗原(CEA)、糖类抗原199(CA19-9)。比较不同组间血清LUCAT1表达水平的差异,分析CRC初诊患者血清中LUCAT1相对表达量与临床病理特征之间的关系以及LUCAT1表达水平与CEA、CA199的关系,并用ROC曲线和约登指数以及曲线下面积(AUC)评估其与CEA、CA199在CRC中的诊断效能。结果:①血清LUCAT1相对表达量在CRC初诊患者中[2.158(1.482~2.977)]显著高于结直肠息肉患者组[1.035(0.771~1.239),U=189.5,P≤0.001]和对照组[0.968(0.782~1.119),U=179,P≤0.001],且术后组[1.010(0.823~1.784),U=214,P≤0.001]显著低于术前水平。②CRC初诊患者血清中LUCAT1相对表达量在不同的肿瘤分化(U=34,P=0.013)、淋巴结转移(U=165,P=0.007)、TNM分期(U=40,P=0.003)差异有统计学意义,而在不同性别、不同年龄中差异无统计学意义(P>0.05)。③CRC初诊患者血清LUCAT1相对表达量与CEA、CA199之间无相关性(r^(2)=0.058,P=0.689;r^(2)=0.206,P=0.148)。④在以健康体检者作为对照作ROC曲线,LUCAT1、CEA、CA199的ROC曲线下面积分别为:0.930、0.807、0.713。LUCAT1的诊断效能优于CEA(Z=2.495,P=0.012)和CA19-9(Z=3.936,P=0.001)。结论:血清LUCAT1相对表达量可能是一个诊断CRC的重要的生物学指标,与疾病的进展相关,在诊断效能上优于传统指标CEA、CA199。
Objective:To investigate the expression level of long non-coding RNA LUCAT1 in colorectal cancer(CRC)and explore the diagnostic value of serum long non-coding RNA LUCAT1 in CRC. Method:Fifty-one patients with CRC diagnosed by pathology,43 cases of colorectal polyps,24 cases of postoperative patients were selected from Rugao People’s Hospital from October 2017 to December 2018.Fifty healthy people were selected as control group.LUCAT1 was quantified by real time fluorescence quantitative PCR.CEA and CA19-9 were detected by chemiluminescence.The difference of serum LUCAT1 level between groups was compared.The relation between serum LUCAT1 level of CRC patients and its clinical pathological characteristics was analyzed.The relation between serum LUCAT1 level of CRC patients and CEA,CA199 was also analyzed.The diagnostic efficacy of them was evaluated by receiver operating characteristic curves(ROC)and Jordan index and area under curve(AUC)in CRC patients. Result:① The relative quantity expression of serum LUCAT1 in CRC group was 2.158(1.482-2.977),significantly higher than that of colorectal polyp group[1.035(0.771-1.239),U=189.5,P≤0.001]and healthy control group[0.968(0.782-1.119),U=179,P≤0.001].The LUCAT1 level of the post surgery group was distinctively smaller than that of the pre surgery group.② The expression level of serum LUCAT1 of CRC patients was significantly different in tumors with different degree of differentiation(U=34,P=0.013),Lymph node metastasis(U=165,P=0.007),TNM stage(U=40,P=0.003).However,there was no significant difference in gender and age(P>0.05).③There was no correlation between serum LUCAT1 level and CEA,CA199(r^(2)=0.058,P=0.689;r^(2)=0.206,P=0.148).④The AUC of serum LUCAT1,CEA and CA199 was 0.930,0.807 and 0.713 respectively when compared with the healthy controls.LUCAT1 was better than CEA(Z=2.495,P=0.012)and CA199(Z=3.936,P=0.001)in diagnostic efficiency.Conclusion:The expression of serum LUCAT1 may serve as an important biological indicator in diagnosing CRC,which may be related to the progress of the disease.It may be better than the traditional index CEA,CA199 in diagnosis efficiency.
