经脐改良单孔腹腔镜手术在妇科良性卵巢肿瘤中的应用效果

Application of Modified Laparoendoscopic Single-site Surgery in the Treatment of Gynecological Benign Ovarian Tumors

目的:探究经脐改良单孔腹腔镜手术治疗妇科良性卵巢肿瘤的效果。方法:选取2018年6月-2020年8月本院收治的良性卵巢肿瘤患者80例作为观察对象,依随机数字表法将其分为试验组和对照组,每组40例。对照组给予传统多孔腹腔镜手术治疗,试验组给予经脐改良单孔腹腔镜手术治疗。比较两组手术相关指标、卵巢功能及并发症发生情况。结果:两组手术时间及术后第一天视觉模拟评分法(visual analogue score,VAS)评分比较,差异均无统计学意义(P>0.05)。试验组术中出血量低于对照组,且术后12 h肛门首次排气例数多于对照组(P<0.05)。两组术后3个月卵巢功能比较,差异均无统计学意义(P>0.05)。两组术后剧烈疼痛、术后3 d内发热例数、术中剔除卵巢囊肿破裂例数、术后月经异常例数比较,差异均无统计学意义(P>0.05)。结论:经脐改良单孔腹腔镜手术在治疗妇科良性卵巢肿瘤中具有手术出血量低、肛门排气较快的优点,且不增加并发症发生率,值得临床推广。

Objective:To investigate the effect of modified laparoendoscopic single-site surgery in the treatment of gynecological benign ovarian tumors.Method:From June 2018 to August 2020,80 patients with benign ovarian tumors admitted to our hospital were selected as observation objects,and they were divided into experimental group and control group according to the random number table method.The control group was treated with traditional multi-port laparoscopic surgery,and the experimental group was treated with modified laparoendoscopic single-site surgery.The surgery related indexes,ovarian function and complications were compared between two groups.Result:There were no statistically significant differences in the duration of surgery and visual analogue score(VAS)on the first day after surgery between two groups(P>0.05).The amount of intraoperative blood loss in experimental group was lower than that in control group,and the number of first anal exhaust 12 h after surgery was higher than that in control group(P<0.05).There were no significant differences in ovarian function between two groups 3 months after surgery(P>0.05).There were no statistically significant differences in the number of severe postoperative pain,fever within 3 d after surgery,ovarian cyst rupture during surgery removed,and menstrual abnormality after surgery between two groups(P>0.05).Conclusion:In the treatment of gynecological benign ovarian tumors,modified laparoendoscopic single-site surgery has the advantages of low blood loss,rapid anal exhaust,and does not increase the incidence of complications,which is worthy of clinical promotion.
