原发性肝癌病人血清hsa_circRNA_103809的表达及临床意义

Clinical significance of serum expression levelof hsa_circRNA_103809 in primary liver cancer

目的探究hsa_circRNA_103809在原发性肝癌病人血清中的表达及其与诊断、治疗和预后的关系。方法选取2016年6月至2019年6月成都市第三人民医院收治的83例病理确诊的原发性肝癌病人设置为肝癌组,75例健康体检者设置为健康对照组。运用实时荧光定量PCR法检测纳入者血清中hsa_circRNA_103809的表达,分析原发性肝癌病人血清中hsa_circRNA_103809表达水平及其与临床病理指标、诊疗和预后的关系。结果原发性肝癌病人血清中hsa_circRNA_103809的表达水平显著高于健康组(P<0.01);hsa_circRNA_103809的表达水平与肿瘤直径、有无门静脉癌栓、淋巴结转移、脏器转移和TNM分期具有明显相关性(P<0.05);原发性肝癌病人手术后血清中hsa_circRNA_103809的表达水平较手术前明显降低(P<0.01);手术后随访1年,原发性肝癌存活组血清hsa_circRNA_103809的表达水平显著低于死亡组(P<0.01)。受试者工作特征(ROC)曲线显示血清hsa_circRNA_103809表达水平对原发性肝癌的诊断效能好,特异度达81.93%,敏感度达85.33%,曲线下面积为0.891。结论血清中的hsa_circRNA_103809与原发性肝癌的发生发展有关,有望成为原发性肝癌早期诊断、疗效和预后评估的重要参考指标。

Objective To explore the serum expression level of hsa_circRNA_103809 in primary liver cancer patients and evaluate its correlation with diagnosis,treatment and prognosis.Methods From June 2016 to June 2019,83 patients with primary liver cancer and 75 healthy individuals receiving regular medical examinations were enrolled and divided into two groups of primary liver cancer and healthy control.The level of hsa_circRNA_103809 was detected by quantitative real time-polymerase chain reaction(PCR)and the correlation analyzed between hsa_circRNA_103809 level with clinical pathological features,diagnosis and prognosis.Results The level of hsa_circRNA_103809 was significantly higher in primary liver cancer group than that in healthy control group(P<0.01).The level of hsa_circRNA_103809 was significantly correlated with tumor size,lymph node metastasis,distant organ metastasis,portal vein tumor thrombosis and TNM stage(P<0.05).Compared with the preoperative level of hsa_circRNA_103809,it decreased significantly after treatment(P<0.01).During 1-year follow-up,the level of hsa_circRNA_103809 was significantly lower in survival group than that in death group(P<0.01).And receiver operating characteristic(ROC)curve analysis revealed that hsa_circRNA_103809 level demonstrated good performance in the diagnosis of primary liver cancer.The specificity was 81.93%,the sensitivity 85.33%and the area under the ROC curve(AUC)0.891.Conclusion Correlated with the development and progression of primary liver cancer,serum level of hsa_circRNA_103809 may act as an important biomarker in the early diagnosis as well as evaluation of efficacy and prognosis of primary liver cancer.
