胃息肉及胃癌组织中PMS2、MSH6、C-met的表达水平及其临床应用价值 被引量:3

摘要 目的对比分析胃息肉及胃癌组织中错配修复基因PMS2、MSH6、细胞间质表皮转化因子(C-met)的表达及其在胃息肉向胃癌转化中的预测价值。方法选取2017年7月至2019年12月安徽理工大学第一附属医院收治的143例胃息肉患者及45例胃癌患者作为研究对象,对比胃息肉及胃癌组织中PMS2、MSH6、C-met的表达水平及其相关性,建立PMS2、MSH6、C-met的受试者工作特征曲线(ROC曲线),分析其在胃息肉向胃癌转化中的预测价值。结果不同病理类型胃息肉及胃癌组织中PMS2、MSH6、C-met阳性表达率比较差异有统计学意义(P<0.01),其中PMS2、MSH6的阳性表达率比较,胃癌组织<腺瘤性息肉<胃底腺息肉<增生性息肉<炎性息肉;C-met阳性表达率比较,胃癌组织>腺瘤性息肉>胃底腺息肉>增生性息肉>炎性息肉。Spearman相关分析结果显示,PMS2、MSH6、C-met表达水平与胃息肉病理类型及病变严重程度呈显著相关性(r=-0.324、r=-0.319、r=0.298,均P<0.001),且PMS2表达与MSH6表达呈正相关(r=0.796,P<0.001),PMS2、MSH6表达与C-met表达呈负相关(r=-0.492、r=-0.483,均P<0.001);ROC曲线分析结果显示,PMS2预测胃息肉向胃癌转化的灵敏度为83.3%、特异度为74.8%,MSH6预测胃息肉向胃癌转化的灵敏度为82.5%、特异度为73.6%,C-met预测胃息肉向胃癌转化的灵敏度为77.8%,特异度为71.2%。结论PMS2、MSH6、C-met表达与胃息肉病理类型及病变严重程度显著相关,可能参与不同病理类型胃息肉向胃癌的发生发展,明确胃息肉的病理类型并检测PMS2、MSH6、C-met表达水平对防治胃息肉向胃癌转化具有重要意义。 Objective To analyze and compare the expression of mismatch repair gene PMS2,MSH6 and cellular mesenchymal epidermal transforming factor(C-met)in gastric polyps and gastric cancer and its predictive value in the transformation of gastric polyps to gastric cancer.Methods A total of 143 cases of gastric polyps and 45 cases of gastric cancer admitted to the First Affiliated Hospital of Anhui University of Science and Technology from Jul.2017 to Dec.2019 were included,and the expression levels and correlations of PMS2,MSH6 and C-met in the gastric polyp and gastric cancer tissues were compared,and receiver operator characteristic(ROC)curves of PMS2,MSH6 and C-met were established,and their predictive value in the transformation of gastric polyp into gastric cancer was analyzed.Results The PMS2,MSH6,C-met positive expression rates in different pathological types of gastric polyps and gastric cancer tissues were statistically significantly different(P<0.01).The PMS2,MSH6 positive rate:gastric carcinoma tissue<adenomatous polyps<fundic gland polyps<hyperplastic polyps<inflammatory polyps;the positive expression rate of C-met:gastric cancer tissues>adenomatous polyps>gastric fundus polyps>proliferative polyps>inflammatory polyps.Spearman correlation analysis showed that the expression levels of PMS2,MSH6 and C-met were significantly correlated with the pathological types and severity of gastric polyps(r=-0.324,r=-0.319,r=0.298,all P<0.001),and the expression levels of PMS2 and MSH6 were positively correlated with the expression of MSH6(r=0.796,P<0.001),while the expressions of PMS2 and MSH6 were negatively correlated with the expression of C-met(r=-0.492,r=-0.483,all P<0.001).ROC curve analysis showed that the sensitivity and specificity of PMS2 to predict the transformation of gastric polyps to gastric cancer were 83.3%and 74.8%,of MSH6 were 82.5%and 73.6%,of C-met were 77.8%and 71.2%,respectively.Conclusion The expressions of PMS2,MSH6 and C-met are significantly correlated with the pathological types and severity of gastric polyps,which may be involved in the occurrence and development of gastric polyps to gastric cancer.It is of great significance to identify the pathological types of gastric polyps and detect the expressions of PMS2,MSH6 and C-met for the prevention and treatment of gastric polyps to gastric cancer.
