三联疗法治疗老年重度环形脱垂混合痔的疗效观察

Observation on Therapeutic Effect of Triplet Surgery for Senior Severe Circumferential Prolapsed Mixed Hemorrhoids

目的观察三联疗法治疗老年重度环形脱垂性混合痔的临床疗效。方法回顾性分析2011年1月至2018年12月收治的60岁以上老年重度环形脱垂混合痔86例患者的临床资料,其中44例为观察组,行自动痔疮套扎术(RPH)联合消痔灵注射加外痔小切口切除三联法治疗,另42例为对照组,行悬吊式外剥内扎术治疗。观察两组临床疗效、手术时间、术中出血量、术后二级以上疼痛、肛缘水肿发生情况。结果两组疗效差异无统计学意义(P>0.05),但观察组患者在手术时间、术中出血量、术后并发症发生率方面均优于对照组(P<0.05)。结论三联疗法对老年重度环形脱垂性混合痔疗效确切,与悬吊式外剥内扎术相比,具有手术时间短、术中出血少、术后并发症少等优点。

Objective This study was to observe the clinical effect of triplet surgery in the treatment of severe circumferential prolapsed mixed hemorrhoids for treating senior type.Method A retrospective analysis of the clinical data of 86 elderly patients with severe circular prolapse and mixed hemorrhoids over 60 years of age who were admitted from January 2011 to December 2018,including 44 cases received the triplet surgery[ruiyun procedure for hemorrhoids(RPH),xiaozhiling injection and radial incision]as the observation group.42 cases received external dissection and internal ligation combined with suspension fixation as control group;then compared both groups following indexes such as total clinical effect,operative time,intraoperative bleeding volume,wound-surface healing time,occurence frequence of overⅡgrade pain,ano-edge edema.Results There was no significant difference in curative effect between the two groups(P>0.05),but patients in the observation group were better than the control group in terms of operating time,intraoperative blood loss,and postoperative complications(P<0.05).Conclusion Results show that this triplet surgery for senior severe prolapsed circumferential mixed hemorrhoids has definite curative effect,and shorter operative time,less intraoperative bleeding volume,less complication than the ones used in the latter.
