雌孕激素序贯治疗药物流产后阴道出血的疗效

Effect of Sequential Therapy of Estrogen and Progesterone on Vaginal Bleeding after Drug Abortion

目的分析雌孕激素序贯治疗药物流产后阴道出血的临床疗效。方法选择我院妇产科于2018年4月至2019年4月收治的药物流产患者110例作为研究对象,按照随机数表法分为两组。对照组55例,在胚囊排出后的第2天进行常规抗感染与止血治疗。观察组55例,在对照组的基础上采用雌孕激素序贯治疗。两组患者均随访2个月,比较两组患者药物流产后的阴道出血量、月经恢复时间、月经量、卵巢功能恢复及并发症发生情况。结果观察组出血量少、出血持续时间≤7 d的比例显著高于对照组(P<0.05),出血量过多、出血时间≥15 d的比例显著低于对照组(P<0.05);观察组月经恢复时间显著低于对照组(P<0.05),复潮后月经持续时间与对照组无明显差异(P>0.05),月经量正常的比例显著高于对照组(P<0.05);观察组药流7 d后的血清E2与P含量改善程度显著高于对照组(P<0.05);观察组并发症发生率为9.09%,显著低于对照组的21.82%(P<0.05)。结论采用雌孕激素序贯治疗药物流产后的阴道出血患者的效果显著,并发症少。

Objective To analyze the clinical effect of sequential treatment with estrogen and progesterone on vaginal bleeding after drug abortion.Methods One hundred and ten cases of patients with medical abortion admitted to the department of obstetrics and gynecology of our hospital from April 2018 to April 2019 were selected as research objects.They were divided into two groups according to the random number table method,and 55 cases were in the control group.Routine anti-infection and hemostatic treatment were conducted on the 2 d after embryo sac expulsion.Observation group of 55 cases,in the control group on the basis of estrogen and progesterone sequential treatment.Patients in both groups were followed up for 2 months to compare the situation of vaginal bleeding,menstrual recovery and menstrual volume,ovarian function recovery and complications after medical abortion in the two groups.Results The proportion of excessive blood loss and bleeding time ≥15 d in the observation group was significantly higher than that in the control group(P<0.05).The proportion of excessive blood loss and bleeding time ≥15 d was significantly lower than that in the control group(P<0.05).The recovery time of menstruation in the observation group was significantly lower than that in the control group(P<0.05),and there was no significant difference between the duration of menstruation after the resumption of menstruation and the control group(P>0.05),and the proportion of normal menstrual volume was significantly higher than that in the control group(P<0.05).The improvement degree of E2 and P in the observation group was significantly higher than that in the control group(P<0.05).The incidence of complications in the observation group was 9.09%,significantly lower than that in the control group(21.82%,P<0.05).Conclusion The use of estrogen and progesterone in the treatment of vaginal bleeding after drug abortion is significant,with few complications.
