负压吸引和外用睾酮霜治疗尿道下裂术后小阴茎的可行性研究

Feasibility Study of Negative Pressure Suction and Topical Testosterone Cream in the Treatment of Small Penis After Hypospadias

目的探讨对尿道下裂术后小阴茎患儿采用负压吸引+外用睾酮霜进行治疗后获得临床效果。方法方便选取该院2017年5月—2020年1月收治的80例尿道下裂术后小阴茎患儿,按数字奇偶法分组,每组40例。治疗组采用负压吸引+外用睾酮霜展开疾病治疗,对照组采用外用睾酮霜展开疾病治疗。就组间阴茎牵伸长度、松弛长度、龟头纵径、龟头横径、患儿家属治疗满意度展开对比。结果治疗前,治疗组牵伸长度为(34.91±7.72)mm,松弛长度为(25.49±6.15)mm,龟头纵径为(13.95±3.16)mm,龟头横径为(14.82±3.41)mm,对照组为(34.85±7.73)、(25.52±6.17)、(13.85±3.19)、(14.83±3.42)mm,组间差异无统计学意义(P>0.05)。治疗后,治疗组牵伸长度为(41.09±8.45)mm、松弛长度为(30.73±6.52)mm、龟头纵径为(15.83±3.53)mm、龟头横径为(16.59±3.43)mm,对照组治疗后分别为(36.25±7.85)、(26.36±7.25)、(14.02±2.11)、(15.02±2.53)mm,差异有统计学意义(t=2.654、2.834、2.784、2.330,P<0.05);治疗组患儿家属治疗满意度为(95.20±2.59)分,高于对照组(72.19±5.23)分,差异有统计学意义(t=24.935,P<0.05)。结论负压吸引+外用睾酮霜的有效应用,可实现尿道下裂术后小阴茎患儿阴茎牵伸长度、松弛长度、龟头纵径、龟头横径有效增加,并且可使患儿家属治疗满意度评价结果显著提升,最终实现尿道下裂术后小阴茎患儿有效预后。

Objective To explore the clinical results obtained after treatment with negative pressure suction+topical testosterone cream in children with small penis after hypospadias.Methods Conveniently selected 80 cases of postoperative hypospadias micropenis children who were admitted to the hospital from May 2017 to January 2020 were divided into groups by digital parity method,with 40 cases in each group.The treatment group was treated with negative pressure suction+topical testosterone cream,and the control group was treated with topical testosterone cream.A comparison was made between the groups on the length of penis stretch,the length of relaxation,the longitudinal diameter of the glans,the transverse diameter of the glans,and the treatment satisfaction of the children's family members.Results Before treatment,the traction length of the treatment group was(34.91±7.72)mm,the slack length was(25.49±6.15)mm,the longitudinal diameter of the glans was(13.95±3.16)mm,and the transverse diameter of the glans was(14.82±3.41)mm.The control group were(34.85±7.73)mm,(25.52±6.17)mm,(13.85±3.19)mm,(14.83±3.42)mm,and there was no statistically significant difference between the groups(P>0.05).After treatment,the treatment group's traction length was(41.09±8.45)mm,slack length was(30.73±6.52)mm,glans longitudinal diameter was(15.83±3.53)mm,glans transverse diameter was (16.59±3.43) mm. After treatment, the control group was (36.25±7.85)mm, (26.36±7.25)mm, (14.02±2.11)mm, (15.02±2.53)mm, the difference was statistically significant (t=2.654, 2.834, 2.784, 2.330,P<0.05);treatment group the treatment satisfaction of the children’s family members was(95.20±2.59) points, which was higher than the control group(72.19±5.23)points, and the difference was statistically significant(t=24.935, P<0.05). Conclusion The effective application of negative pressure aspiration + topical testosterone cream can effectively increase penile extension, relaxation length, glans longitudinal diameter and glans transverse diameter of children with small penis after hypospadias, and can increase the satisfaction of the children’s family;evaluation results are significantly improved, and finally an effective prognosis for patients with small penis after hypospadias surgery is realized.
