儿童后颅窝髓母细胞瘤与室管膜瘤MRI鉴别诊断的术前研究

MRI Differential Diagnosis of Medulloblastoma and Ependymoma in Posterior Cranial Fossa in Children

目的:选用MRI平扫+增强扫描技术,探讨儿童后颅窝室管膜瘤与髓母细胞瘤的影像学鉴别诊断对术前的应用价值。方法:选2016.12~2020.05区间泉州市妇幼保健院(泉州医学高等专科学校教学合作医院)经手术治疗的21例儿童后颅窝室管膜瘤与髓母细胞瘤患者研究,术前均实施MRI平扫+增强扫描技术,以术后病理证实情况回顾性分析MRI诊断结果,并对儿童后颅窝室管膜瘤与髓母细胞瘤ADC均值、囊性成分所占比例、脑室系统扩张发生率、四叠体上抬发生率进行统计学分析。结果:儿童后颅窝室管膜瘤与髓母细胞瘤ADC均值差异、囊性成分所占比例差异有统计学意义,四叠体上抬发生率差异、脑室系统扩张发生率无统计学意义。结论:MRI平扫+增强扫描技术在儿童后颅窝室管膜瘤与髓母细胞瘤鉴别诊断中有极高价值,可为临床术前提供帮助。

Objective:To evaluate the value of MRI in the diagnosis of posterior fossa Ependymoma and Medulloblastoma in children.Methods:A total of 21 children with posterior fossa Ependymoma and Medulloblastoma underwent surgical treatment from December 2016 to May 2020 in Quanzhou Women’s And Children’s Hospital(Quanzhou Medical College Vocational School), all patients were performed with plain and enhanced MRI before operation. The MRI findings were analyzed retrospectively, the ADC mean value, the proportion of cystic components, the incidence of ventricular system dilatation and the incidence of tetrad up-lift were analyzed statistically.Results:There were significant differences in ADC values and the proportion of cystic components between Ependymoma and Medulloblastoma in posterior fossa of children, but no significant differences in the incidence of Tetrad uplifts and ventricular system dilation.Conclusion:MRI combined with contrast-enhanced scanning is of great value in the differential diagnosis of Ependymoma and Medulloblastoma in posterior fossa of children.
