慢性呼吸系统疾病老年住院患者衰弱与用药依从性的现状调查及相关性分析

Investigation and correlation analysis of frailty and medication compliance in elderly hospitalized patients with chronic respiratory diseases

目的通过调查了解慢性呼吸系统疾病老年住院患者衰弱与用药依从性的现状,并进一步探讨二者之间的相关性。方法采取便利抽样法,通过患者一般资料调查表、Tilburg衰弱评估量表及Morisky服药依从性量表对2018年11月至2019年6月入住北京某医院呼吸科的150例慢性呼吸系统疾病老年患者进行衰弱与用药依从性的现状调查;并通过Pearson相关分析、偏相关分析及线性回归对其相关性进行分析。结果用药依从性差(<6分)的患者为117例(78%),衰弱状态(≥5分)的患者为115例(76.7%),均处于较高水平。用药依从性得分[(4.30±1.98)分]与衰弱得分[(5.79±2.28)分]成负相关(P<0.05),衰弱程度越高,用药依从性越差。衰弱各维度对患者用药依从性的影响程度由大到小依次为身体衰弱、心理衰弱和社会衰弱。结论通过衰弱评估可初步预测患者的用药依从性,医务人员可根据患者衰弱维度的不同为患者制定有针对性的用药指导方案及护理措施,从而提高患者的用药依从性。

Objective Investigate the status of frailty and medication compliance in elderly hospitalized patients with chronic respiratory diseases,and further explore the relationship between them.Methods Conduct a survey of 150 elderly patients with chronic respiratory diseases who were admitted to the respiratory department of a hospital in Beijing from November 2018 to June 2019 by the convenience sampling method through the general patient information questionnaire,the Tilburg frailty assessment scale,and the Morishy medication compliance scale.Results The proportion of patients with poor medication compliance was 117(78%),and the incidence of frailty was 115(76.7%),both at a high level.Medication compliance score(4.30±1.98)was negatively correlated with frailty score(5.79±2.28)(P<0.05),the higher the degree of frailty,the poorer the medication compliance.The degree of impact of the various dimensions of frailty on the patient's medication compliance in descending order is physical frailty,mental frailty and social frailty.Conclusion The frailty assessment can preliminarily predict the patient's medication compliance,and medical staff can formulate a targeted medication guide plan and nursing measures for the patient according to the different dimensions of the patient's frailty,thereby improving the patient's medication compliance.
