多层螺旋CT(MSCT)、MRI在诊断侵袭性血管黏液瘤(AAM)中价值分析

Value Analysis of Multislice Spiral CT(MSCT)and MRI in the Diagnosis of Invasive Angiomyxoma(AAM)

目的:探讨多层螺旋CT(MSCT)、MRI在侵袭性血管黏液瘤(AAM)诊断方面的应用价值。方法:回顾分析本院2016年6月~2019年6月收治的10例AAM患者,所有患者均实施多层螺旋CT诊断、MRI诊断,且患者均经过手术病理证实,总结AAM患者影像学检查特点。结果:10例AAM患者经过CT平扫增强检查,动脉期病灶均表现出明显强化,以"散在细条状强化"与静脉期、实质期"环状、弧样"表现为主;增强检查后病灶血管明显强化;MRI检查后,以T1WI低信号、T2WI高信号为主,病灶强化表现出"旋涡状"征象,病灶边缘较为模糊。10例AMM患者均实施手术治疗,术前栓塞治疗可降低手术治疗难度,术后短期复发率较高。结论:MSCT与MRI在AAM患者诊断方面各有优缺点,通过使用MSCT与MRI联合诊断,可获取较多的病灶信息,并依据典型的影像学资料表现为疾病的治疗以及术后随访提供依据,最大程度帮助患者恢复健康。

Objective:To investigate the value of multislice spiral CT(MSCT)and MRI in the diagnosis of invasive angiomyxoma(AAM).Methods:10 patients with AAM admitted to our hospital from June 2016 to June 2019 were retrospectively analyzed.All the patients were diagnosed by multi-slice spiral CT and MRI,and all the patients were confirmed by surgery and pathology,summarize the characteristics of imaging examination of AAM patients.Results:After CT plain scan enhancement examination,10 AAM patients showed significant enhancement in arterial stage lesions,with"scattered thin strip enhancement"and"ring and arc"as the main manifestations.After the enhancement examination,the blood vessels in the lesion were significantly strengthened.After MRI examination,low T1WI signal and high T2WI signal were dominant,and the lesion enhancement showed"vortex-like"signs,and the edge of the lesion was fuzzy.All the 10 AMM patients underwent surgical treatment,and preoperative embolization therapy could reduce the difficulty of surgical treatment,with a high postoperative short-term recurrence rate.Conclusion:Both MSCT and MRI have advantages and disadvantages in the diagnosis of AAM patients.Through the combined diagnosis of MSCT and MRI,more lesion information can be obtained,and the typical imaging data can provide a basis for the treatment of the disease and postoperative follow-up,so as to help patients recover to the greatest extent.
