颞下颌关节张闭口位磁共振成像在颞下颌关节盘前移位中的诊断价值 被引量:2

摘要 目的:分析颞下颌关节张闭口位磁共振成像(MRI)在颞下颌关节盘前移位中的诊断价值。方法:回顾性分析我院2018年6月—2020年8月诊断为颞下颌关节盘不可复性前移位的120例(240侧颞下颌关节)患者,根据临床初期诊断分为不可复性颞下颌关节盘前移位组40例(108侧)、可复性颞下颌关节盘前移位组40例(76侧)、无颞下颌关节盘移位组40例(56侧),将三组MRI核磁共振检查结果诊断与临床初期诊断进行比较,对两种诊断的差异性进行统计学分析。结果:通过对240侧颞下颌关节使用MRI技术进行诊断后,经统计学分析,可复性颞下颌关节盘前移位组76侧颞下颌关节MRI诊断结果与临床初期诊断有显著差异,有统计学意义(P<0.05),不可复性颞下颌关节前移位组与无颞下颌关节盘移位组两种诊断结果无显著差异,无统计学意义(P>0.05)。结论:将颞下颌关节张闭口位核磁共振成像诊断技术应用在颞下颌关节盘前移位中具有较高的诊断价值,可有效区分其可复性及不可复性关节盘前移位情况,特别是针对可复性颞下颌关节盘前移位的早期判断,在后续针对性治疗及效果评估上都具有较高的临床应用价值。 Objective To analyze the diagnostic value of MRI in temporomandibular joint anterior disc displacement.Methods Retrospective analysis was performed on 120 patients(240 TMJ)diagnosed with irreversible anterior displacement of the TMJ disc in our hospital from June 2018 to August 2020.According to the preliminary clinical diagnosis is divided into an irreversible temporomandibular joint disc before shift group of 40 cases(108 sides),hone the temporomandibular joint disc before shift group of 40 cases(76 sides),without the temporomandibular joint disc displacement group of 40 cases(56),will be three sets of MRI. MRI results show,the early diagnosis and clinical diagnosis for statistical analysis of the differences between two kinds of diagnosis.Results After 240 sides of TMJ were diagnosed by MRI technology,statistical analysis showed that the MRI diagnosis results of 76 sides of TMJ in the reducible TMJ anterior disc displacement group were significantly different from the initial clinical diagnosis(P﹤0.05),and there was no significant difference between the two groups,There was no statistical significance(P﹥0.05).Conclusion The application of MRI in temporomandibular joint anterior disc displacement has high diagnostic value,which can effectively distinguish between reducible and irreducible anterior disc displacement,especially for the early judgment of reducible anterior disc displacement,which has high clinical application value in the follow-up targeted treatment and effect evaluation value.
