独活寄生汤治疗腰椎管狭窄症临床有效性研究

Clinical study on Duhuo Jisheng Decoction in treatment of lumbar spinal stenosis

目的观察腰椎管狭窄症患者采用独活寄生汤治疗的临床效果。方法将我院收治的150例腰椎管狭窄症患者随机分为对照组(常规对症治疗)与观察组(常规对症+独活寄生汤治疗)各75例。观察两组患者腰椎功能与疼痛度、血清炎性反应递质和抗炎细胞因子水平以及血液流变学参数等。结果治疗后,观察组日本骨科协会(JOA)评估量表评分高于对照组,视觉模拟评分法(VAS)评分低于对照组(均P<0.05);两组患者白介素-6(IL-6)与肿瘤坏死因子-α(TNF-α)水平均有所下降,白介素-4(IL-4)与白介素-10(IL-10)水平均有所上升且观察组变化幅度均大于对照组(P<0.05);两组患者全血黏度高、中、低切及血浆黏度、纤维蛋白原、红细胞比容等指标水平均有明显改善且观察组改善效果均显著优于对照组(P<0.05)。结论独活寄生汤治疗腰椎管狭窄症,可明显提高患者炎症反应与血液流变学的改善效果,有效缓解疼痛程度,提升腰椎功能获得更理想的治疗有效性。

Objective To observe the effect of Duhuo Jisheng Decoction in treatment of lumbar spinal stenosis(LSS).Methods A total of 150 patients with LSS were randomly divided into control group(conventional symptomatic treatment)and observation group(conventional symptomatic treatment+Duhuo Jisheng Decoction) with 75 cases each.The lumbar function and pain degree,serum inflammatory response transmitter and anti-inflammatory cytokine levels,and hemorheological parameters were observed.Results After treatment,the Japanese Orthopaedic Association(JOA) assessment scale of observation group was higher than control group,and the Visual analogue scale(VAS)score was lower than control group(all P<0.05);the levels of interleukin-6(IL-6),tumor necrosis factor-α(TNF-α) decreased,and IL-4 and IL-10 increased,and the changes of observation group were greater than control group,the amplitudes were larger than those of control group(P<0.05);there were significant improvements in the levels of high,medium,and low-cut whole blood viscosity plasma viscosity,fibrinogen,hematocrit and other indicators in both groups,and were more significant in observation group than control group(P<0.05).Conclusion Duhuo Jisheng Decoction can significantly improve the inflammatory response and blood rheology of the patients with LSS,effectively relieve the pain degree,and improve the lumbar spine function to obtain a more ideal treatment effectiveness.
