艾塞那肽用于治疗早期糖尿病肾病的临床疗效分析

Clinical Effect of Exenatide in the Treatment of Early Diabetic Nephropathy

目的分析艾塞那肽用干早期糖尿病肾病(early diabetic nephropathy,EDN)患者中的治疗效果。方法选取2018年11月-2019年8月于解放军第九六0医院就诊的EDN患者88例,随机分为观察组和对照组各44例。对照组采取口服降糖药+皮下注射胰岛素治疗;观察组采用口服降糖药+皮下注射艾塞那肽治疗,对比两组治疗效果。结果两组治疗前后随机尿白蛋白与肌酐比值、血清胱抑素C、甘油三酯、总胆固醇、体质指数均低干治疗前,且观察期低干对照组,差异有统计学意义(P<0.05)。两组治疗前后空腹血糖、餐后2 h血糖、糖化血红蛋白、收缩压水平均明显低于治疗前,差异有统计学意义(P<0.05),但组间差异无统计学意义(P>0.05)。两组治疗前后血清肌酐差异无统计学意义(P>0.05),且组间差异无统计学意义(P>0.05)。结论艾塞那肽可减少EDN患者尿液白蛋白排泄,延缓肾病的进程。

Objective To analyze the therapeutic effect of exenatide in patients with early diabetic nephropathy(EDN).Methods A total of 88 patients with EDN treated in No.960 Hospital of People's Liberation Army of China from November 2018 to August 2019 were randomly divided into observation group and control group.The control group was treated with oral hypoglycemic drugs and subcutaneous injection of insulin,and the observation group was treated with oral hypoglycemic drugs andsubcutaneous injection of exenatide.The therapeutic effects of the two groups were compared.Results The ratio of urinary albumin to creatinine,serum cystatin C,triglyceride,total cholesterol and body mass index in the two groups before and after treatment were significantly lower than those before treatment,and the observation period was significantly lower than that in the control group(P<0.05).The fasting blood glucose,2 h postprandial blood glucose,glycosylated hemoglobin and systolic blood pressure in the two groups after treatment were significantly lower than those before treatment(P<05),but there was no significant difference between the two groups(P>0.05).There was no significant difference in serum creatinine between the two groups before and after treatment,and there was no significant difference between the two groups(P>0.05).Conclusion Exenatide can reduce urinary albumin excretion and delay the process of nephropathy in patients with EDN.
