中医结合康复治疗脑卒中偏瘫肩痛研究进展

Research progress of traditional Chinese medicine combined with rehabilitation treatment for shoulder pain of stroke patients with hemiplegia

目前,相关临床研究发现脑卒中的发病率明显增加,威胁患者的生命健康。脑卒中偏瘫肩痛在临床较常见,严重影响患者生活质量和上肢运动功能。在临床中,偏瘫患者上肢运动功能恢复是一个比较慢的过程,肩痛不仅会增加患者痛苦,还会延缓和阻碍其上肢运动功能的恢复,加重偏瘫病情。中医治疗脑卒中偏瘫肩痛的方法主要有针灸和推拿,常用于偏瘫恢复期和后遗症期,针对患者肌肉和神经进行刺激,能够促使患侧血液循环,改善肢体功能,减轻疼痛,从而改善患者肩部疼痛、肌肉紧张和神经功能。现代康复治疗主要有良肢摆放、低频电疗、肌肉起止点疗法等,可单独使用,也可联合使用。中医联合现代康复治疗在改善脑卒中偏瘫肩痛患者神经功能、肌肉活动能力、肩痛等方面的疗效明显。本文综述了目前中医结合现代康复治疗脑卒中偏瘫肩痛的相关进展,为其临床治疗提供参考。

At present,relevant clinical studies have found that the incidence of stroke is significantly increasing,threatening the life and health of patients.Stroke hemiplegic shoulder pain is common in clinic,which seriously affects the quality of life and upper limb motor function of patients.In clinical practice,the upper limb motor function recovery of hemiplegic patients is a relatively slow process,shoulder pain will not only increase the pain of patients,but also delay and hinder the recovery of upper limb motor function,aggravating the condition of hemiplegia.Traditional Chinese medicine treatment methods of stroke hemiplegic shoulder pain mainly include acupuncture and massage,commonly used in hemiplegic recovery period and sequelae period,with muscle and nerve stimulation for patients,can promote the blood circulation of affected side,improve limb function,reduce pain,so as to improve patients with shoulder pain,muscle tension and nerve function.Modern rehabilitation therapy mainly includes good limb placement,low-frequency electrotherapy,muscle starting and ending point therapy,etc.,which can be used alone or in combination.Traditional Chinese medicine combined with modern rehabilitation therapy has obvious effect on improving neurological function,muscle activity and shoulder pain of stroke patients with hemiplegia.This paper reviews the current progress of traditional Chinese medicine combined with modern rehabilitation in the treatment of stroke hemiplegic shoulder pain,and provides reference for its clinical treatment.
