无创机械通气对慢性阻塞性肺疾病合并呼吸衰竭患者血气指标的影响

The Effect of Noninvasive Mechanical Ventilation on Blood Gas Index of Patients With Chronic Obstructive Pulmonary Disease and Respiratory Failure

目的探讨分析慢性阻塞性肺疾病合并呼吸衰竭患者经无创正压通气的临床治疗效果和血气指标变化情况。方法研究纳入我院从2018年3月—2020年3月收治的98例慢性阻塞性肺疾病合并呼吸衰竭患者,应用随机数字表法进行分组,包括对照组(n=49,行常规治疗)和观察组(n=49,以常规治疗为基础行无创正压通气治疗),比较两组治疗效果、血气指标。结果对照组取得治疗总有效率为75.51%,观察组取得治疗总有效率为95.92%,差异具有统计学意义(P<0.05)。动脉血氧分压(PaO2)、pH值以及二氧化碳分压(PaCO2)等血气指标对照组和观察组比较,PaO2、pH值差异无统计学意义(P>0.05),PaCO2观察组低于对照组,差异具有统计学意义(P<0.05)。结论无创正压通气能够有效治疗慢性阻塞性肺疾病合并呼吸衰竭患者,对患者的血气指标具有良好的改善作用,有利于患者病情的康复。

Objective To explore and analyze the clinical therapeutic effect and blood gas index changes of patients with chronic obstructive pulmonary disease complicated with respiratory failure through noninvasive positive pressure ventilation.Methods 98 patients with chronic obstructive pulmonary disease complicated with respiratory failure treated in our hospital from March 2018 to March 2020 were divided into control group(n=49,routine treatment)and observation group(n=49,noninvasive positive pressure ventilation based on routine treatment).The treatment effects and blood gas indexes of the two groups were compared.Results the total effective rate was 75.51%in the control group and 95.92%in the observation group(P<0.05).Arterial oxygen partial pressure(PaO2),pH value and carbon dioxide partial pressure(PaCO2)were compared between the control group and the observation group,there was no significant difference in PaO2 and pH(P>0.05),but PaCO2 in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion noninvasive positive pressure ventilation can effectively treat patients with chronic obstructive pulmonary disease complicated with respiratory failure,improve the blood gas indexes of patients,and is conducive to the rehabilitation of patients.
