人工气道精细化护理管理在机械通气治疗重症患者中的应用效果 被引量:6

Effect of artificial airway refined nursing management in the treatment of severe patients with mechanical ventilation

摘要 目的:探讨人工气道精细化护理管理在机械通气治疗重症患者中的应用效果。方法:选取2017年2月至2020年1月临床上收治的78例机械通气重症患者分为干预组和对照组。对照组给予常规护理,干预组给予人工气道精细化护理管理,比较两组患者呼吸机相关肺炎及误吸发生情况,干预前后GPIS评分、机械通气时间、ICU入住时间、住院时间。结果:干预组患者呼吸机相关肺炎、误吸发生率分别为2.56%、0.00%,较对照组的20.51%、15.38%显著降低(P<0.05)。干预前两组患者GPIS评分无显著性差异(P>0.05);干预后两组患者GPIS评分均显著降低(P<0.05),且干预组患者GPIS评分改善更显著(P<0.05)。干预组患者机械通气时间、ICU入住时间、住院时间较对照组患者显著缩短(P<0.05)。结论:精细化护理管理应用于机械通气重症患者可降低呼吸机相关肺炎发生,缩短治疗时间,改善预后。 Objective:To explore and analyze the application effect of artificial airway refined nursing management in the treatment of severe patients with mechanical ventilation.Methods:78 patients with severe mechanical ventilation who were clinically admitted from February 2017 to January 2020 were selected and divided into intervention group and the control group.The control group was given routine care,and the intervention group was given artificial airway refined care management.The occurrence of ventilator-related pneumonia and aspiration,GPIS scores before and after intervention,mechanical ventilation time,ICU Check-in time,length of hospital stay were compared between the two groups.Results:The incidence of ventilator-associated pneumonia and aspiration of patients in the intervention group were 2.56%and 0.00%respectively,which were significantly lower than 20.51%and 15.38%in the control group(P<0.05).There was no significant difference in GPIS score between the two groups before the intervention(P>0.05).After the intervention,the GPIS scores of the two groups were significantly reduced(P<0.05).The GPIS score of the intervention group improved significantly(P<0.05).The mechanical ventilation time,ICU stay time,and hospital stay of the intervention group were significantly shorter than those in the control group(P<0.05).Conclusion:The application of refined nursing management to severely ill patients with mechanical ventilation can reduce the occurrence of ventilator-related pneumonia,shorten treatment time and improve prognosis.
