从中医心功能失调探讨肠易激综合征病机

Discussion on the pathogenesis of irritable bowel syndrome from heart dysfunction

肠易激综合征(IBS)是最常见的功能性肠病,患病率高,是中医药诊治的优势病种之一。IBS病位在肠,与五脏功能失调相关,肝郁脾虚是其核心病机,治疗以疏肝健脾为主。但近年来心功能失调对IBS发生发展的影响备受关注,文章论述了心主血脉功能失调可引起IBS肠道症状、心神失调是IBS精神心理症状发生的关键、心神失调与IBS内脏高敏感性联系密切、小肠功能异常可引起心功能失调而致IBS症状和从心论治IBS疗效确切等内容,希望丰富IBS病机,助益IBS辨证论治。

Irritable bowel syndrome(IBS) is the most common functional bowel disease with high prevalence, which is also one of the dominant diseases in the diagnosis and treatment of traditional Chinese medicine. It is generally acknowledged that IBS is located in the intestine and is related to the dysfunction of the five internal organs. Liver depression and spleen deficiency is the core pathogenesis of IBS, and the treatment is mainly to soothing the liver and strengthening the spleen. However, in recent years, much more attention has been paid to the influence of heart dysfunction on the occurrence and development of IBS. This paper discusses that heart governing blood and vessels dysfunction can bring IBS intestinal symptoms;the disorder of heart spirit is the key to the occurrence of IBS mental symptoms;the disorder of heart spirit is closely related to IBS visceral hypersensitivity,small intestinal dysfunction can cause heart dysfunction, thus causing IBS symptoms;Treatment of IBS from heart is efficient. We hope to enrich the pathogenesis of IBS and then help syndrome differentiation and treatment of IBS.
