摘要
目的着眼于医院及医生对医生手术能力评价的需求,探索外科医生手术能力定量化评价模型在医院管理实践的应用。方法专题小组讨论了课题组研制的骨科医生手术能力评价模型应用于其他科室时的条件及调整方案,并用大数据模拟分析法测算了神经外科科室127位医生的手术能力分值、在职称评价和医生个性化能力水平可视化表达的应用。结果模型应用于神经外科时,需要对模型中的主要手术能力分值和调整系数值进行调整;调整后的模型测算127位神经外科科室主任医师、副主任医师、主治医师、住院医师的手术能力均值分别是429.65分、376.31分、153.34分、87.32分;当医生的手术能力分值及月均主刀数量分别达到134.98~194.37分、0.00~0.06台,191.63~234.65分、0.24~3.11台,252.49~369.41分、3.77~17.05台,278.53~472.75分、19.33~40.42台时,认为其手术能力水平分别达到了住院医师、主治医师、副主任医师、主任医师的水平。结论研制的手术能力定量化评价模型可以应用于其他外科科室,计算的不同职称级别医生标准手术能力水平为医生职称晋升提供了可视化参考,为医院职称评审改革及绩效管理等工作提供借鉴。
Objective Focused on the needs of hospitals and doctors for the evaluation of doctors’surgical capabilities,the application of the quantitative evaluation model of surgeons’surgical capabilities in hospital management practices is explored.Methods The special group method is used to clarify the conditions and adjustment plans of the developed orthopaedic surgeon’s surgical competency evaluation model when applied to other departments.The big data simulation analysis method is used to calculate the surgical competency scores of 127 doctors in the neurosurgery department,as well as the application of the evaluation of professional titles and the visual expression of the doctor’s personalized competency level.Results Appling the model in neurosurgery causes the adjustment of main surgical competency scores and adjustment coefficient values.The adjusted model estimates that the average surgical capabilities of 127 neurosurgery department chief physicians,deputy chief physicians,attending physicians,and residents are 429.65,376.31,153.34,and 87.32,respectively.The doctor’s surgical competency score reached134.98~194.37,191.63~234.65,252.49~369.41,and 278.53~472.75 respectively.Monthly average number of surgeons reached 0.00~0.06,0.24~3.11,3.77~17.05,and 19.33~40.42.It is believed that the level of surgical competency has reached the level of the resident physician,attending physician,deputy chief physician,and chief physician.Conclusion The developed quantitative evaluation model of surgical competency can be applied to other surgical departments.The calculated standard surgical competency level of doctors with different professional titles provides a visual reference for the promotion of professional titles of doctors and provides a reference for the reform of professional title review and performance management in hospitals.
