摘要
目的建立西安地区21~30岁女性AMH及基础性激素的参考范围,调查本地区21~30岁育龄期女性卵巢功能情况,为相关部门生育政策的调整及年轻女性生育计划的制定提供理论依据。方法以2019年6月至2019年12月进行体检的21~30岁的健康女性230例为研究对象,用Beckman Coulter UniCel DxI 800全自动化学发光分析仪检测血清AMH及基础性激素(E_(2)、FSH、LH、PRL、P、T)水平。比较不同年龄组女性的AMH及基础性激素水平;分析21~30岁女性AMH、E_(2)、FSH、LH、PRL、P、T与年龄的相关性;分析21~30岁女性AMH与E_(2)、FSH、LH、PRL、P、T的相关性。结果西安地区21~25岁、26~30岁女性AMH的参考范围分别为0.70~10.01 ng/mL、0.63~10.07 ng/mL。在21~30岁年龄段,AMH随年龄增长有一定程度的下降,但与年龄无显著相关性(P>0.05);PRL、P与年龄呈负相关(P<0.05);E_(2)、FSH、LH随年龄增长有一定程度的升高,T随年龄增长有一定程度的降低,但均与年龄无显著相关性(P>0.05)。21~30岁女性中,AMH与FSH、FSH/LH呈显著负相关(P<0.05);AMH与LH呈显著正相关(P<0.05);AMH与P、T、E_(2)和PRL无显著相关性(P>0.05)。结论21~30岁育龄女性卵巢功能随年龄下降不明显,但个体差异大,临床医生应给予育龄女性建立专属的卵巢功能档案,采用AMH结合FSH、LH及FSH/LH多种指标判断卵巢功能。
Objective To establish the reference range of AMH and basic sex hormones in women aged 21-30 in Xi'an,and to investigate the ovarian function of women aged 21-30 in this area,so as to provide theoretical basis for the adjustment of fertility policy of relevant departments and the formulation of fertility plan for young women.Methods A total of 230 healthy women aged 21-30 who underwent physical examination from June 2019 to December 2019 were selected as the study objects,and the serum AMH and basic sex hormones(E_(2),FSH,LH,PRL,P,T)were detected by Beckman Coulter UniCel DxI 800 automatic chemiluminescence analyzer.The levels of AMH and basic sex hormones in women of different age groups were compared;the correlation between AMH,E_(2),FSH,LH,PRL,P,T and age in women aged 21-30 were analyzed;the correlation between AMH and E_(2),FSH,LH,PRL,P and T in women aged 21-30 were analyzed.Results The reference ranges of AMH for women aged 21-25 and 26-30 in Xi'an are 0.70-10.01 ng/mL and 0.63-10.07 ng/mL respectively.In the age group of 21-30 years old,AMH decreased with age,but there was no significant correlation between AMH and age(P>0.05);there were significant negative correlation between PRL,P and age(P<0.05);E_(2),FSH and LH increased with age,and T decreased with age,but there were no significant correlation between E_(2),FSH,LH and T with age(P>0.05).In women aged 21-30,there were significant negative correlation between AMH and FSH and FSH/LH(P<0.05);there was significant positive correlation between AMH and LH(P<0.05);there were no significant correlation between AMH and P,T,E_(2) and PRL(P>0.05).Conclusion In women of reproductive age from 21 to 30,the decline of ovarian function is not significant with age,but the individual difference is large.Clinicians should establish a special ovarian function profile for women of reproductive age,and use AMH combined with FSH,LH and FSH/LH indicators to determine ovarian function.
