炎症因子表达在不宁腿综合征中的应用研究

Application of inflammatory factor expression in restless legs syndrome

目的探讨对不宁腿综合征(RLS)患者检测炎症因子水平的临床意义。方法通过对46例不宁腿综合征患者(RLS组)、46例健康者(HC组)进行多导睡眠监测,国际不宁腿综合征严重程度量表(IRLS)、匹茨堡睡眠质量指数量表(PSQI)、7项广泛性焦虑障碍量表(GAD-7)评估及检测C-反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平,并进行组间比较。结果RLS组睡眠效率(62.25±9.28)%较HC组(81.94±6.27)%低、入睡潜伏期(46.84±9.32)min较HC组(21.93±5.51)min长、觉醒次数(32.63±7.62)次较HC组(20.66±4.41)次多,且差异均有统计学意义(P<0.05)。RLS组周期性睡眠腿动指数(PLMI)、IRLS、PSQI、GAD-7明显高于HC组,差异均有统计学意义(P<0.05)。RLS组血清CRP、IL-6、TNF-α水平都高于HC组,且差异均有统计学意义(P<0.05)。RLS患者PSQI与PLMI存在正相关关系,r值为0.426,P<0.05。结论推测患者血清CRP、IL-6和TNF-α的水平可成为一种新的、简单的临床判断RLS参考指标之一。

Objective To investigate the clinical significance of detecting the level of inflammatory factors in patients with restless legs syndrome(RLS).Methods 46 patients with restless legs syndrome(RLS group)and 46 healthy controls(HC group)were monitored by polysomnography.Their levels of C-reactive protein(CRP),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)were measured by International Restless Legs Syndrome Severity Scale(IRLS),Pittsburgh Sleep Quality Index(PSQI),and Generalized Anxiety Disorder Scale-7(GAD-7),respectively.The results were compared between two groups.Results The sleep efficiency(62.25±9.28)%of the RLS group was lower than that(81.94±6.27)%of the HC group.The sleep latency(46.84±9.32)min of the RLS group was longer than that(21.93±5.51)min of the HC group.And the awakening times(32.63±7.62)of the RLS group were more than those(20.66±4.41)of the HC group.All the above differences were statistically significant(P<0.05).The RLS group had significantly higher periodic legs movement index(PLMI)during sleep as well as significantly higher scores of IRLS,PSQI and GAD-7 than the HC group,and the differences were statistically significant(P<0.05).The levels of serum CRP,IL-6 and TNF-αin the RLS group were higher than those of the HC group,and the differences were statistically significant(P<0.05).There was a positive correlation between PSQI and PLMI in patients with RLS,with r value being 0.426,P<0.05.Conclusion The prediction of serum CRP,IL-6 and TNF-αlevel in patients can be a new and simple kind of reference index for clinical judgment of RLS.
