摘要
目的探讨无创呼吸机联合沙丁胺醇对慢性阻塞性肺疾病(COPD)并Ⅱ型呼吸衰竭患者血清炎性因子与CK-MB活性的影响。方法选择2018年5月-2020年4月该院收治的COPD并Ⅱ型呼吸衰竭患者86例,以随机数表法将其分为两组。对照组43例应用无创机械通气治疗,研究组43例在此基础上应用沙丁胺醇气雾剂治疗。对比两组治疗的总有效率,治疗前后炎性因子[C反应蛋白(CRP)、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]与肌酸激酶同工酶(CK-MB)水平,以及不良反应情况。结果在治疗的总有效率对比中,研究组95.35%较对照组79.07%更高,差异有统计学意义(χ^(2)=5.108,P<0.05)。治疗后,研究组CRP(7.8±1.3)mg/L、TNF-α(14.5±5.3)pg/L、IL-6水平(11.3±3.2)pg/mL较对照组(21.4±4.0)mg/L、(25.0±6.9)pg/L、(16.5±4.0)pg/mL更低,差异有统计学意义(t=21.204、7.914、6.657,P<0.05)。治疗后,研究组CK-MB水平(5.8±1.6)U/L较对照组(10.6±1.2)U/L更低,差异有统计学意义(t=15.738,P<0.05)。在不良反应发生率对比中,研究组9.30%与对照组4.65%对比差异无统计学意义(χ^(2)=0.179,P>0.05)。结论无创呼吸机联合沙丁胺醇在COPD并Ⅱ型呼吸衰竭患者中具有显著的疗效,且改善了血清炎性因子与CK-MB活性。
Objective To investigate the effect of non-invasive ventilator combined with salbutamol on serum inflammatory factors and CK-MB activity in patients with chronic obstructive pulmonary disease(COPD)and typeⅡrespiratory failure.Methods Eighty-six patients with COPD and type II respiratory failure admitted to the hospital from May 2018 to April 2020 were selected and divided into two groups by random number table method.43 cases in the control group were treated with non-invasive mechanical ventilation,and 43 cases in the study group were treated with salbutamol aerosol on this basis.To compare the total effective rate of the two groups of treatments,inflammatory factors[C-reactive protein(CRP),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)]and creatine kinase isoenzymes before and after treatment(CK-MB)levels and adverse reactions.Results In the comparison of the total effective rate of treatment,95.35%of the study group was higher than 79.07%of the control group,the difference was statistically significant(χ^(2)=5.108,P<0.05).After treatment,the study group's CRP(7.8±1.3)mg/L,TNF-α(14.5±5.3)pg/L,and IL-6 levels(11.3±3.2)pg/mL were compared with the control group(21.4±4.0)mg/L,(25.0±6.9)pg/L,(16.5±4.0)pg/mL were lower,the difference was statistically significant(t=21.204,7.914,6.657,P<0.05).After treatment,the CK-MB level(5.8±1.6)U/L of the study group was lower than that of the control group(10.6±1.2)U/L,the difference was statistically significant(t=15.738,P<0.05).In the comparison of the incidence of adverse reactions,there was no statistically significant difference between 9.30% of the study group and 4.65% of the control group(χ^(2)=0.179,P>0.05).Conclusion Non-invasive ventilator combined with salbutamol has a significant effect in patients with COPD and type II respiratory failure,and it improves the serum inflammatory factors and CK-MB activity.
