个性化心理护理对高位复杂性肛瘘患者围手术期焦虑、抑郁、疼痛及术后恢复的影响

Influence of Personalized Psychological Nursing on Perioperative Anxiety,Depression,Pain and Postoperative Recovery of Patients with High Complex Anal Fistula

目的研究个性化心理护理在高位复杂性肛瘘围手术期中对患者焦虑、抑郁、疼痛及术后恢复的影响。方法选取2018年7月至2019年7月长垣市中医医院收治的100例高位复杂性肛瘘患者为研究对象,按随机数表法分为对照组和试验组,各50例。对照组接受常规心理护理,试验组在对照组基础上接受个性化心理护理。比较两组患者的心理状态、疼痛情况、应对能力、希望水平、健康调查简表(SF-36)评分及恢复情况。结果护理前,两组患者心理状态、疼痛、应对能力、希望水平、SF-36评分比较,差异无统计学意义(P>0.05)。护理后,试验组心理状态评分、疼痛评分低于对照组,应对能力、希望水平、SF-36评分高于对照组,住院时间短于对照组,住院费用低于对照组(P<0.05)。两组患者创面愈合时间比较,差异无统计学意义(P>0.05)。结论在高位复杂性肛瘘患者围手术期中提供个性化心理护理,可对患者焦虑、抑郁、疼痛、生活质量与术后恢复产生积极影响,值得推广。

Objective To study the effect of personalized psychological nursing on anxiety,depression,pain and postoperative recovery of patients with high complex anal fistula.Methods A total of 100 patients with high complex anal fistula who were admitted to Changyuan Hospital of Traditional Chinese Medicine from July 2018 to July 2019 were selected as the research objects.According to the random number table,they were divided into control group and experimental group,with 50 cases in each group.The control group received routine psychological nursing and the experimental group received personalized psychological nursing on the basis of the control group.The psychological status,pain status,coping ability,hope level,36-item short-form(SF-36)score and recovery status of the two groups were compared.Results There was no statistical difference in psychological status score and pain score between the two groups before nursing(P>0.05).After nursing,the psychological status score and pain score of the experimental group were lower than those of the control group,and the coping ability,hope level and SF-36 score were higher than those of the control group,and the length of stay was shorter than that of the control group,and hospitalization expenses was lower than that of the control group(P<0.05).There was no statistical difference in wound healing time between the two groups(P>0.05).Conclusion Providing personalized psychological nursing in the perioperative period of patients with high complex anal fistula have a positive impact on patients’anxiety,depression,pain,quality of life and postoperative recovery,and it is worth popularizing.
