子宫内膜息肉患者宫腔镜术后发生宫腔粘连的影响因素

Influencing factors of intrauterine adhesions in patients with endometrial polyps after hysteroscopic surgeries

目的:探讨子宫内膜息肉患者宫腔镜术后发生宫腔粘连的影响因素。方法:回顾性分析2018年1月至2021年1月该院收治的60例行宫腔镜手术治疗的子宫内膜息肉患者的临床资料,统计宫腔粘连发生情况,对可能导致子宫内膜息肉患者宫腔镜术后发生宫腔粘连的相关因素进行单因素及Logistic回归分析。结果:60例子宫内膜息肉患者宫腔镜术后发生宫腔粘连14例(23.33%);单因素分析结果显示,子宫内膜息肉患者宫腔镜术后发生宫腔粘连与体质量指数、手术时间、绝经、内膜增生、剖宫产史、宫内节育器、结扎、糖尿病、高血压、贫血等均无相关性(P>0.05),与息肉类型、刮宫史、盆腔炎、人工流产史等明显相关(P<0.05);Logistic回归分析结果显示,多发息肉、刮宫史、盆腔炎、人工流产史均为子宫内膜息肉患者宫腔镜术后发生宫腔粘连的影响因素(P<0.05)。结论:多发息肉、刮宫史、盆腔炎、人工流产史均为子宫内膜息肉患者宫腔镜术后发生宫腔粘连的影响因素。

Objective:To explore influencing factors of intrauterine adhesions in patients with endometrial polyps after hysteroscopic surgeries.Methods:A retrospective analysis of the clinical data of 60 patients with endometrial polyps treated by hysteroscopic surgeries in the hospital from January 2018 to January 2021 was performed.The occurrence of intrauterine adhesions was statistically analyzed.Univariate factor and Logistic regression analysis were performed on the related factors that may lead to the occurrence of intrauterine adhesions.Results:In the 60 patients with endometrial polyps,14 cases(23.33%)had intrauterine adhesions after hysteroscopic surgeries.Univariate analysis showed that intrauterine adhesions was not significantly correlated with body mass index,operation time,menopause,endometrial hyperplasia,history of cesarean section,intrauterine device,ligation,diabetes,hypertension,anemia and the like.(P>0.05),and was significantly correlated with polyp type,history of curettage,pelvic inflammatory disease and history of artificial abortion(P<0.05).Logistic regression analysis showed that multiple polyps,history of curettage,pelvic inflammatory disease,and history of artificial abortion were all influential factors for the occurrence of intrauterine adhesions in the patients with endometrial polyps after hysteroscopic surgeries(P<0.05).Conclusions:Multiple polyps,history of curettage,pelvic inflammatory disease,and history of artificial abortion were all influential factors for the occurrence of intrauterine adhesions in the patients with endometrial polyps after hysteroscopic surgeries.
