江西省社区居民基层就诊满意度及其影响因素分析

Analysis on the Satisfaction of Community Residents at Grassroots of Jiangxi Province and Its Influencing Factors

目的:全面了解江西省社区居民在基层医疗机构就诊满意度现状,分析其主要影响因素,为实现基层医疗机构的高质量、可持续发展提供参考。方法:采用分层随机抽样方法,对有基层医疗机构就诊经历的社区居民进行问卷调查。运用单因素方差分析和独立样本检验比较社区居民基层就医满意度在不同组间的分布差异,采用二分类logistic回归分析影响社区居民基层就诊满意度的因素。结果:江西省社区居民在基层医疗机构就诊满意度总均分为分,1107例(70.70%)社区居民基层医疗机构就诊满意度总分较低(<24分)。其中,社区居民对基层医生满意度平均分为,对基层医疗机构满意度平均分为。不同民族、婚姻状况、学历、现居住地及身体健康状况的社区居民就诊满意度比较有统计学差异(P<0.05)。现居住地、是否患有慢性病及身体健康状况是影响江西省社区居民就诊满意度的主要因素(P<0.05)。结论:江西省地处中部,相对全国来说,基层医疗卫生服务总体上仍面临供需失衡问题,社区居民基层就诊满意度较低。建议提高县域医疗服务能力,打造城乡医联体,从而改善基层就诊环境,完善家庭医生签约服务,以提高社区居民基层就诊满意度,促进基层医疗机构建设,加快分级诊疗的实现。

Objective:To comprehensively understand the status quo of community residents’satisfaction with visits to primary medical institutions in Jiangxi Province,analyze the main influencing factors,and provide references for realizing the high-quality and sustainable development of primary medical institutions.Methods:The stratified random sampling method was used to conduct a questionnaire survey among community residents who had medical experience in primary medical institutions.A one-way analysis of variance and an independent sample test were used to compare the distribution differences of community residents’primary medical satisfaction among different groups,and binary logistic regression was used to analyze the factors affecting community residents’primary medical satisfaction.Results:The total average satisfaction score of community residents in the treatment of primary medical institutions in Jiangxi province was(20.87±3.90)points,and 1107 cases(70.7%)of community residents in primary medical institutions had a low total score of satisfaction(<24 points).Among them,the community residents’satisfaction with primary doctors are(10.45±2.16)points,and their satisfaction with primary medical institutions are(10.43±2.11)points.Community residents of different nationalities,marital status,educational background,current residence and physical health status showed statistical differences in the satisfaction with medical visits(P<0.05).Current residence,chronic disease and physical health are the main factors affecting the satisfaction of community residents in Jiangxi Province(P<0.05).Conclusion:Located in the central part of China,primary medical and health services in Jiangxi were still facing an imbalance between supply and demand.Community residents have low satisfaction with treatment at the grassroots level.Therefore,it is recommended to improve county medical service capacity and create an urban-rural medical consortium to improve the grassroots consultation environment and improve family doctors’contracted services to improve community residents’grassroots consultation satisfaction,promote the construction of grassroots medical institutions,and accelerate the realization of graded diagnosis and treatment.
