摘要 目的探讨临床微生物检验和细菌耐药性监测的应用价值。方法选择我院2016年1月至2019年6月收治的600例临床微生物检验和细菌耐药性检验标本作为本次研究对象,用全自动微生物鉴定仪进行细菌鉴定,自动化仪器法进行药敏试验。检测用的抗生素主要有青霉素、氨苄西林、头孢曲松、头孢唑林、环丙沙星、苯唑西林、美罗培南、庆大霉素及万古霉素,统计检验结果并比较。结果600例病原菌中革兰阳性球菌77例(12.84%),革兰阴性杆菌523例(87.16%)。数据显示,革兰阴性杆菌对氨苄西林、头孢曲松、美罗培南有较高耐药性,耐药率可达到92.93%、41.87%、8.80%,而革兰阳性球菌则对青霉素、氨苄西林有较高耐药性,耐药率分别为92.21%、67.53%。两组间进行比较,差异有统计学意义(P<0.05)。结论临床上微生物检验中耐药菌株的检出率较高,抗生素耐药现状较为严峻,需要临床医师引起重视,并进一步加强抗菌药物管理,提高抗菌药物使用的规范性和安全性,严格遵守相关规定规范使用抗生素,保证患者用药有效性和医疗安全。 Objective To explore the application value of clinical microbiology test and bacterial drug resistance monitoring.Methods Six hundred clinical microbiological and bacterial resistance samples admitted to our hospital from January 2016 to June 2019 were selected as the objects of this study.Bacteria were identified by automatic microbiological identification instrument,and drug sensitivity test was conducted by automatic instrument method.The main antibiotics used in the detection were penicillin,ampicillin,ceftriaxone,cefazolin,ciprofloxacin,benzacillin,meropenem,gentamicin and vancomycin.The test results were analyzed and compared.Results Among the 600 pathogenic bacteria,77 were gram-positive cocci(12.84%)and 523 were gram-negative bacilli(87.16%).The data showed that gram-negative bacilli had high resistance to ampicillin,ceftriaxone and menopenem,with resistance rates of 92.93%,41.87%and 8.80%,while gram-positive cocci had high resistance to penicillin and ampicillin,with resistance rates of 92.21%and 67.53%,respectively.The difference between the two groups was statistically significant(P<0.05).Conclusion Clinically drug-resistant strains of microbiological detection rate is higher,antibiotic drug resistance situation is relatively serious,clinicians need to attention,and further strengthen the management of antimicrobial drugs,improve the safety of antimicrobial drug use normative and strictly abide by the relevant provision standard use of antibiotics and ensure the safety of the medical and effectiveness for treating patients.
