摘要
目的探讨鼻内镜下等离子射频消融术对慢性肥厚性鼻炎患者鼻腔生理功能及TIgE、ECP水平的影响。方法选择2019年1月至2020年10月我院收治的80例慢性肥厚性鼻炎患者作为研究对象,根据治疗方案将其分为对照组和观察组,各40例。对照组接受传统鼻甲切除术治疗,观察组采用鼻内镜下等离子射频消融术治疗。比较两组的鼻腔生理功能恢复情况、应激反应指标(TIgE、ECP)、鼻内镜Lund Kennedy评分、鼻腔鼻窦结局测试-20(SNOT-20)评分及ICAM-1、TXB2、白三烯水平。结果术前,两组的纤毛运动速率、鼻腔阻力、鼻腔通气面积无显著差异(P>0.05);术后7 d,观察组的纤毛运动速率、鼻腔通气面积大于对照组,鼻腔阻力小于对照组(P<0.05)。术前,两组的TIgE、ECP水平无显著差异(P>0.05);术后2 d,观察组的TIgE、ECP水平低于对照组(P<0.05)。术前,两组的Lund Kennedy、SNOT-20评分无显著差异(P>0.05);术后7 d,观察组的Lund Kennedy、SNOT-20评分低于对照组(P<0.05)。术前,两组的ICAM-1、TXB2、白三烯水平无显著差异(P>0.05);术后7 d,观察组的ICAM-1、TXB2、白三烯水平低于对照组(P<0.05)。结论鼻内镜下等离子射频消融术用于慢性肥厚性鼻炎患者的临床治疗中,可促进鼻腔生理功能恢复,调节机体TIgE、ECP、ICAM-1、TXB2及白三烯水平,提高患者的生活质量,值得推广。
Objective To explore the effects of plasma radiofrequency ablation under nasal endoscopy on the physiological function of the nasal cavity and the levels of TIgE and ECP in patients with chronic hypertrophic rhinitis.Methods A total of 80 patients with chronic hypertrophic rhinitis treated in our hospital from January 2019 to October 2020 were selected as the research objects.According to the treatment scheme,the patients were divided into control group and observation group,with 40 cases in each group.The control group was treated with traditional turbinectomy,and the observation group was treated with plasma radiofrequency ablation under nasal endoscope.The recovery of nasal physiological function,stress response indexes(TIgE,ECP),Lund Kennedy score of nasal endoscopy,sino-nasal outcome test-20(SNOT-20)score,ICAM-1,TXB2 and leukotriene levels were compared between the two groups.Results Before operation,there were no significant differences in ciliary movement rate,nasal resistance and nasal ventilation area between the two groups(P>0.05);at 7 d after operation,the ciliary movement rate and nasal ventilation area in the observation group were greater than those in the control group,and the nasal resistance was lower than that in the control group(P<0.05).Before operation,there were no significant differences in the levels of TIgE and ECP between the two groups(P>0.05);at 2 d after operation,the levels of TIgE and ECP in the observation group were lower than those in the control group(P<0.05).Before operation,there were no significant differences in the scores of Lund Kennedy and SNOT-20 between the two groups(P>0.05);at 7 d after operation,the scores of Lund Kennedy and SNOT-20 in the observation group were lower than those in the control group(P<0.05).Before operation,there were no significant differences in the levels of ICAM-1,TXB2 and leukotriene between the two groups(P>0.05);at 7 d after operation,the levels of ICAM-1,TXB2 and leukotriene in the observation group were lower than those in the control group(P<0.05).Conclusion Plasma radiofrequency ablation under nasal endoscope in the clinical treatment of patients with chronic hypertrophic rhinitis can promote the recovery of nasal physiological function,regulate the levels of TIgE,ECP,ICAM-1,TXB2 and leukotriene,and improve the quality of life of patients,which is worthy of promotion.
