Burow's楔形推进皮瓣对面部皮肤肿瘤切除术后皮肤缺损及伤口美观度的影响

Evaluation of Burow’s Wedge Advanced Flap for Repairing Skin Defects after Facial Skin Tumor Resection

目的:探讨Burow's楔形推进皮瓣对面部皮肤肿瘤切除术后皮肤缺损及伤口美观度的影响。方法:选取2017年9月-2019年2月笔者医院收治的80例面部皮肤肿瘤切除术后皮肤缺损患者,均采用Burow's楔形推进皮瓣修复治疗,并使辅助切口顺应面部分区自然轮廓线,观察术后修复效果和并发症发生率,随访术后18个月患者复发率,并评价伤口美观满意度。结果:术后80例患者皮片和皮瓣全部成活,创面均I期愈合。随访1~18个月,2例患者失访,5例患者于术后1年复发,再次行根治性切除术;其余73例患者面部外形轮廓及视觉效果良好,修复组织与周围皮肤在色泽、质地、轮廓等方面相同,切口线相对隐蔽、切口瘢痕平软,无继发性缺损与畸形及感染。伤口美观度满意度为97.37%。结论:Burow's楔形推进皮瓣治疗面部皮肤肿瘤切除术后皮肤缺损疗效显著,安全性较高,伤口美观度较好。

Objective To investigate the effect of Burow’s wedgeadvanced skin flap on skin defectand wound aesthetics after resection of facial skin tumor.Methods 80 patients with skin defects after facial skin tumor resection in our hospital from September 2017 to February 2019 were selected.All patients were treated with Burow’s wedge-shaped advancement flap,and the auxiliary incision was made to conform to the natural contour of the facial region,The postoperative repair effect and complication rate were observed,the recurrence rate of patients 18 months after follow-up was observed,and the satisfaction of wound aesthetics was evaluated.Results All the skin flaps and flaps survived and the wounds healed in the first stage.During the follow-up period of 1-18 months,2 patients were lost to follow-up,5 patients recurred one year after operation and underwent radical resection again;the facial contour and visual effect of the remaining 73 patients were good,and the repaired tissue was the same as the surrounding skin in color,texture and contour,the incision line was relatively hidden,the incision scar was soft,and there was no secondary defect,deformity and infection.Wound aesthetic satisfaction was 97.37%.Conclusion Burow’s wedge advanced flap is effective in treating skin defects after resection of facial skin tumor,with high safety and good wound appearance.
