摘要
目的探讨乙酰半胱氨酸联合盐酸氨溴索在婴幼儿肺炎中的应用效果及对患儿通气状态、IFN-γ及LTE4水平的影响。方法选取2019年12月至2021年1月我院接收的82例婴幼儿肺炎患儿为研究对象,以抽签法将其分为对照组和观察组,每组41例。对照组给予常规对症治疗+盐酸氨溴索注射液,观察组在对照组基础上给予吸入用乙酰半胱氨酸溶液。比较两组患儿治疗前、后通气状态指标、IFN-γ、LTE4、免疫功能指标与肺功能指标水平及不良反应发生情况。结果治疗后,两组的PaO_(2)、SaO_(2)均升高,PaCO_(2)均降低,且观察组显著优于对照组(P<0.05)。治疗后,两组的IFN-γ水平均升高,LTE4水平均降低,且观察组显著优于对照组(P<0.05)。治疗后,两组的CD4^(+)、CD4^(+)/CD8^(+)均升高,CD8^(+)均降低,且观察组显著优于对照组(P<0.05)。治疗后,两组的FEV_(1)%、FEV_(1)/FVC、PEF均升高,且观察组高于对照组(P<0.05)。观察组的不良反应总发生率显著低于对照组(P<0.05)。结论乙酰半胱氨酸联合盐酸氨溴索在婴幼儿肺炎中的应用效果显著,不仅可有效改善通气状态,调节机体IFN-γ、LTE4水平,还能提高机体免疫功能与肺功能,减少不良反应的发生。
Objective To explore the application effect of acetylcysteine combined with ambroxol hydrochloride in infants with pneumonia and its influences on ventilation status,IFN-γ and LTE4 levels of children.Methods A total of 82 infants with pneumonia received in our hospital from December 2019 to January 2021 were selected as the research objects and divided into control group and observation group by lottery method,with 41 cases in each group.The control group was given routine symptomatic treatment+ambroxol hydrochloride injection,and the observation group was given acetylcysteine solution for inhalation on the basis of the control group.The levels of ventilation status indexes,IFN-γ,LTE4,immune function indexes and pulmonary function indexes before and after treatment and adverse reactions were compared between the two groups.Results After treatment,PaO_(2) and SaO_(2) increased and PaCO_(2) decreased in the two groups,and those in the observation group were significantly better than the control group(P<0.05).After treatment,the level of IFN-γincreased and the level of LTE4 decreased in the two groups,and those in the observation group were significantly better than the control group(P<0.05).After treatment,CD4^(+),CD4^(+)/CD8^(+) increased and CD8^(+) decreased in the two groups,and those in the observation group were significantly better than the control group(P<0.05).After treatment,the FEV_(1)%,FEV_(1)/FVC and PEF in the two groups increased,and those in the observation group were significantly higher than the control group(P<0.05).The total incidence of adverse reactions in the observation group was significantly lower than that in the control group(P<0.05).Conclusion The acetylcysteine combined with ambroxol hydrochloride applied in infants with pneumonia has remarkable effect.It can not only effectively improve the ventilation status and regulate the levels of IFN-γ and LTE4,can also improve immune function and pulmonary function,reduce the occurrence of adverse reactions.
