大黄、芒硝外敷促进二次剖宫产术后腹部切口愈合的疗效分析

Effect of external application of rhubarb and mirabilite on abdominal incision healing after secondary cesarean section

目的探讨二次剖宫产术后腹部切口外敷大黄、芒硝的临床疗效。方法选取200例行二次剖宫产术产妇,按照随机数字表法分为对照组与研究组,各100例。对照组应用常规微波护理,研究组在实施常规微波护理基础上应用大黄、芒硝外敷腹部切口。比较两组产妇术后切口愈合效果、产后不同时间温哥华瘢痕评估量表(VSS)、视觉模拟评分法(VAS)评分及术后并发症发生情况。结果研究组术后甲级切口愈合率为92.00%,明显高于对照组的80.00%,差异有统计学意义(P<0.05)。产后24h,两组产妇VSS、VAS评分比较,差异无统计学意义(P>0.05);产后10d,两组产妇VSS、VAS评分均明显低于产后24 h,且研究组明显低于对照组,差异有统计学意义(P<0.05)。研究组术后并发症发生率为4.00%,明显低于对照组的14.00%,差异具有统计学意义(P<0.05)。结论二次剖宫产手术产妇采用大黄、芒硝外敷腹部切口,有利于切口愈合,可降低术后并发症,减轻疼痛感,值得临床应用。

Objective To explore the clinical effect of external application of rhubarb and mirabilite on abdominal incision after secondary cesarean section.Methods 200 pregnant women who underwent secondary cesarean section were randomly divided into control group and study group,with 100 cases in each group.The control group was treated with routine microwave nursing,and the study group was treated with rhubarb and mirabilite on the basis of routine microwave nursing.The postoperative wound healing effect,Vancouver Scar Assessment Scale(VSS),visual analogue scale(VAS)and postoperative complications were compared between the two groups.Results The healing rate of grade a incision in the study group was 92.00%,which was significantly higher than 80.00% in the control group(P<0.05).At 24 hours postpartum,there was no significant difference in the scores of VSS and vas between the two groups(P>0.05);At 10 days postpartum,the scores of VSS and vas in the two groups were significantly lower than those at 24 hours postpartum,and the study group was significantly lower than that in the control group(P<0.05).The incidence of postoperative complications in the study group was 4.00%,which was significantly lower than 14.00% in the control group(P<0.05).Conclusion The external application of rhubarb and Glaubefs salt to the abdominal incision in the second cesarean section is conducive to the healing of the incision,reduce the postoperative complications and reduce the sense of pain,which is worthy of clinical application.
