多奈哌齐联合司来吉兰对阿尔茨海默病患者认知功能及不良反应的影响

Effect of Donepezil Combined with Selegilan on Cognitive Function and Adverse Reactions in Patients with Alzheimer’s Disease

目的探究多奈哌齐联合司来吉兰对阿尔茨海默病(AD)患者认知功能及不良反应的影响。方法选取2019年7月-2021年1月解放军联勤保障部队第九八三医院收治的98例AD患者,依据随机数字表法分为对照组与观察组,各49例。对照组给予多奈哌齐治疗,观察组应用多奈哌齐联合司来吉兰治疗,比较两组AD评定量表(ADAS)评分、长谷川痴呆量表(HDS)评分、蒙特利尔认知评估量表(MoCA)评分、简易精神状态检查量表(MMSE)评分、血清生化指标[β样淀粉蛋白(Aβ)、超氧化物歧化酶(SOD)、丙二醛(MDA)]及不良反应情况。结果观察组治疗后ADAS评分低于对照组,HDS评分高于对照组,差异有统计学意义(P<0.05);两组MoCA、MMSE评分均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05);观察组治疗后Aβ、MDA水平低于对照组,SOD水平高于对照组,差异有统计学意义(P<0.05);观察组不良反应发生率为12.24%,与对照组的8.16%比较,差异无统计学意义(P>0.05)。结论多奈哌齐联合司来吉兰可有效减轻AD患者的病情程度,改善其认知功能,降低氧化应激指标,且不增加不良反应风险。

Objective To explore the effects of donepezil combined with selegiline on cognitive function and adverse reactions in patients with Alzheimer’s disease(AD).Methods A total of 98 AD patients admitted to the 983 Hospital of the PLA Joint Logistic Support Force from July 2019 to January 2021 were selected and divided into the control group and the observation group according to the random number table method,with 49 cases in each group.The control group was treated with donepezil,while the observation group was treated with donepezil combined with selegiline.The scores of Alzheimer’s disease assessment scale(ADAS),Hasegawa dementia scale(HDS),Montreal cognitive assessment scale(MoCA),mini mental state examination scale(MMSE),serum biochemical indexes[β-amyloid protein(Aβ),superoxide dismutase(SOD),malondialdehyde(MDA)]and adverse reactions were compared between the two groups.Results After treatment,the ADAS score of the observation group was lower than that of the control group,and the HDS score was higher than that of the control group,the difference was statistically significant(P<0.05).The MoCA and MMSE scores of the two groups were higher than those before treatment,and the observation group was higher than the control group,the difference was statistically significant(P<0.05).After treatment,the levels of Aβ and MDA in the observation group were lower than those in the control group,and the SOD level was higher than that in the control group,the difference was statistically significant(P<0.05).The incidence of adverse reactions in the observation group was 12.24%,which was compared with 8.16% in the control group,the difference was not statistically significant(P>0.05).Conclusion Donepezil combined with selegilan can effectively reduce the severity of AD patients,improve their cognitive function,reduce oxidative stress indexes,and do not increase the risk of adverse reactions.
